Lists of aviation accidents

On September 14, 2008, a scheduled aircraft of the Russian airline Aeroflot , which was operated by its subsidiary Aeroflot-Nord and was on its way from Moscow to Perm , had an accident on Aeroflot flight 821 . Perm is located on the western foothills of the Ural Mountains . The Boeing 737-500 ( aircraft registration VP-BKO ) crashed during the approach to the Perm airport at Bolshoye Savino at around 05:20 a.m. local time (13 September; 23:20 p.m. UTC ), killing all 88 occupants of the aircraft.

causes

According to the investigation report, the crash can be traced back to a chain of causes consisting of pilot errors, inadequate training, inadequate maintenance and deficiencies in the management of Aeroflot-Nord:

  • The immediate cause of the accident was the spatial disorientation of the crew (especially the master), which led to an abnormal flight position , a steep descent and ultimately to the impact. The spatial disorientation occurred while the aircraft was being flown at night, in clouds and with the autopilot and throttle off. The lack of sufficient skills for flight control, a lack of crew resource management and a lack of skills in escaping from abnormal flight situations using "western" attitude indicators such as are available in foreign and modern Russian-made aircraft have contributed to the disorientation . These display devices differ from those found in the types of aircraft previously flown by both pilots ( Tupolev Tu-134 , Antonov An-2 ).
  • The systemic cause of the accident was the inadequate operational management by the airline in the areas of flight operations and aircraft maintenance.
  • Deficiencies in aircraft maintenance led to several flights with large thrust asymmetry between the two engines (with the same thrust lever position) being carried out before the flight involved in the accident, even though this asymmetry far exceeded the permissible limit. This technical problem led to an increased workload on the crew during the approach, especially since it did not eliminate the thrust asymmetry, which would have been possible by separately regulating the two engines.
  • The captain's forensic examination revealed that there was alcohol in his body. The flight assignments performed by the master in the days before the accident had also led to fatigue and were also not in accordance with the national regulations on flight duty times.

background

The flight was actually a flight operated by the parent company Aeroflot , but was operated by Aeroflot-Nord under a reciprocal agreement on joint passenger transport. In the international media, however, there is mostly simply talk of an Aeroflot flight. For this reason, it spoke of considerable damage to the image of Aeroflot by Aeroflot-Nord and announced that it would forbid the subsidiary from using the designation “Aeroflot” in the company name with immediate effect.

The plane crashed on a track on the Trans-Siberian Railway ; Nobody was harmed on the ground, only railway systems were destroyed over a length of 500 m.

Among the victims were 21 foreigners as well as the Russian presidential advisor General Gennady Troschew , who was involved in both Chechnya wars.

The plane

The Boeing 737-500 entered service in September 1992. Previously it was operated by the Chinese airlines Xiamen Airlines , China Southwest Airlines and Air China .

Aeroflot-Nord had leased the aircraft from Pinewatch Limited for the period July 28, 2008 to March 21, 2013.

consequences

memorial

Aeroflot's insurance company paid the bereaved families of the victims up to 2 million rubles (about 55,000 euros) per fatality. In addition, the families received 12,000 rubles (about 330 euros) in damages from the Russian state.

Because of the plane crash, Aeroflot immediately terminated its cooperation with Aeroflot-Nord and announced that it would immediately cease joint flights with Aeroflot-Nord and that it will prohibit the use of Aeroflot aircraft for these flights. From September 15, 2008, Aeroflot-Nord was no longer allowed to use the Aeroflot brand. Aeroflot-Nord refused to bow to this decision.

Aeroflot also hired a commission to review the aircraft of another subsidiary, Aeroflot-Don.

On September 25, 2008, all Boeing 737 flights in Russia were suspended by the Russian supervisory authority until further notice. They could only be resumed after the pilots had received additional simulator training. It is believed that the crew misinterpreted the cockpit displays.

Damage to the railway line

The aircraft fell on the double-track section Bacharewka-Perm II of the Sverdlovsk Railway . The track bed of the railway system was destroyed over a length of 100 m and around 500 m of the underground railway communication network.

Normal train traffic on the Trans-Siberian Railway was interrupted; the trains were routed via a diversion line via the Chusovskaya station (in the city of Chusovoy ). Because of the search work at the crash site, the repair work on the railway line was temporarily postponed. On the evening of September 14, 2009, train traffic was fully resumed on the regular route.
