Scott Wheeler, a deep cover agent, wakes up with another prisoner, Marco, to find himself aboard a retrofitted nuclear-powered submarine in a CIA black site. With the help of a rookie agent and Marco, he must escape and find out who locked him up.

He goes through his most recent memories. Wheeler is a CIA agent who, along with his partner, Melissa Ballard, is looking for a leak in the CIA using a USB drive that requires two components to activate. Melissa carries the drive while Wheeler carries the activation key. The next day the two are attacked. While he tries to escape, Melissa is apparently killed. Wheeler manages to escape. He contacts his dealers to meet with him. At the meeting he finds this dead. Wheeler is then captured and drugged by Agent Ferris, who believes Wheeler was responsible for the deaths of his CIA colleagues. Agent Rhodes, on the other hand, believes Wheeler is innocent. They bring Wheeler to the submarine for interrogation, where they meet the crew of the submarine. The submarine crew is split between those who operate the submarine, led by Captain Darrow, the security crew led by Kingsley and CIA agents Cassie Taylor and Ellis Ryan. Wheeler then wakes up and goes back to the beginning of the film.

Wheeler is interrogated by Agent Preston but does not reveal any information. Agent Rhodes then takes over the interrogation of Wheeler because they want to know where the activation key is. Wheeler realizes that Rhodes is a traitor and Rhodes kills the rest of the CIA crew, forcing the rest of the submarine's security team to obey his orders. Wheeler manages to escape, but is captured by Cassie and Ellis. They plan to go to the control room to contact someone from above, not knowing that the submarine needs to rise to the surface or the buoy needs to float. When they reach the control room, they are ambushed and managed to escape, but Ellis is killed in an exchange of fire.

Wheeler and Taylor force the crew to raise the submarine by increasing the pipe pressure. Taylor is shot on the way through the submarine, but survives. Taylor learns that Rhodes has recruited Wheeler for the CIA and that he wants to sell the drive, which contains an algorithm to activate sleep agents. They get help by freeing Marco who also knows what is going on and therefore cannot be killed. Rhodes instructs Kagan to go to "Plan B" while convincing the submarine crew to stay at their current depth by telling them Wheeler is a traitor for allying with Kingsley.

Wheeler, Taylor and Marco ambush Kingsley and his crew, killing everyone but Kingsley. Kingsley is at Marco's mercy. Wheeler and Taylor meet Rhodes and the captain and try to clear up the lies of Rhodes. Rhodes panics and captures Captain Darrow. Taylor is captured by Kagan. Agent Ballard is revealed to be allied with Rhodes after faking her death. Ballard wants to take Wheeler with him to restore the activation key as she, Rhodes and Kagan make millions from it. In an unnoticed moment, Wheeler switches on the phone and its loudspeaker in the submarine so that the extraction team knows what is going on. Rhodes orders Ballad to shoot Taylor. Wheeler steps in and says that if they let Taylor go, he'll tell them where the activation key is kept. While Taylor believes him, Rhodes ticks off and a shootout breaks out. Kagan and the submarine crew kill each other. Ballard shoots Wheeler and escapes instead of killing Taylor even though Wheeler is holding her. Wheeler then kills Rhodes.

Wheeler and Taylor leave the submarine and are caught by the CIA. They are told that the CIA cannot open a file because the submarine is a black place and cannot admit their guilt as it would show their incompetence. The CIA continues to search for Ballard and instructs Wheeler and Taylor to join forces and look for Rhodes buyers. It is revealed that Marco escaped the submarine, leaving Kingsley alive, naked and shawl tied. Marco pursues and kills Ballard and lets Wheeler know that they are now even.
