The opening scene shows an apocalyptic Philadelphia destroyed by a civil war in 2024 with a variation of the flag of the United States .

In 1988, several people in Philadelphia died within a short period of time with the same symptoms: their brains dissolving and they bleed from every opening in their head. The police officer Thomas Lockhart discovers three puncture holes in the neck of the victims and hopes to be promoted to detective with this case . His brother-in-law, Detective Holt, realizes that the pattern of injuries suggests serial murder . When Lockhart and his partner Maddox were able to question an injured victim before they died, the attacker described them as an African-American woman in a blue hooded coat. A mass manhunt starts immediately, Lockhart and Maddox discover the suspect. The chase ends at the Girard Broad Street Line station. When Lockhart confronts the suspect, she talks about his unborn daughter and then dies under an approaching train. A little later, Lockhart's wife dies in childbirth.

Nine years later, on the anniversary, a copycat seems to be continuing the series of murders. Lockhart, meanwhile promoted to detective, fetches the files from the archives. Among the evidence from 1988 are keys for a small aircraft that was only manufactured in 1996. When the police officers leave the precinct to follow the lead, the physicist Naveen Rao approaches them and tries in vain to convey his theory of time travel in connection with the phases of the blood moon . The airplane key leads Lockhart and his partner to an airfield; There he meets the same perpetrator who died in front of his eyes in 1988, but has not aged. When Maddox tries to arrest her, she shoots him and takes Lockhart hostage. She pushes him off the plane into the sea; he escapes to the beach, where the plane lies in burning rubble, while she disappears without a trace.

In 2006 Lockhart left the police force. He lives in his car and is obsessed with time travel. His daughter is now under the care of her uncle Holt, who urges Lockhart to seek psychiatric help. Instead stolen Lockhart, the badge of Holt and thus arrives at the addresses of the widow of one of the victims. From this he learns that the deceased had led a racist, nationalist militia and spread corresponding propaganda. In 2015, the physicist Rao, who was believed to have disappeared, tested a method for remote-controlled killing after an injection on pigs. He kidnaps Lockhart to prevent this from disrupting the perpetrator's series of murders, as this averts greater evil. Lockhart escapes Rao and confronts the murderer who turns out to be his granddaughter Rya. She explains that Lockhart himself trained Rya for her mission after the terror of the militia sparked a civil war. Rya travels back in time and completes her targeted killings, averting the civil war suggested at the beginning.
