The Río Sama rises in the Peruvian Western Cordillera . There it has its origin in the small lake Laguna Cotanvilque, which is about 4680  m high . From there it initially flows 20 km to the south. At river kilometer 152 the river is dammed by the Jarumas dam ( ). Later the river turns east. It passes the district administration center Tarata and takes the tributaries Río Aruma from the left and later the Río Salado from the right. The Río Sama turns gradually to the south-west and south-south-west and reaches the desert-like coastal plain at river kilometer 52. He crosses this in a south-southwest direction and finally reaches the Pacific coast. The Panamericana (1S) crosses the river at Tomasiri at river kilometer 43. Irrigated agriculture takes place in sections along the course of the river.
