action

While holding a crying baby, a woman fights and kills a man, presumably her husband. Four years later, Angela and Brian have a four-year-old daughter named Cora. The three are a happy, close family. Both are doctors, although Angela is on leave to be a full-time mom. A friend of Angela's, Linda, introduces Angela to Katie. Katie's four-year-old daughter Maddie gets along with Cora. Katie says she left an abusive husband but doesn't elaborate on it. She also refuses to be photographed. Brian's mother, Donna, doesn't like Katie from the start and questions her background because she senses something is wrong with her.

Angela confides in Katie that she had problems conceiving, so she used an egg donor for Cora and is now pregnant again. Devastated after a miscarriage, she invites Katie to move into the guest house and be a part-time nanny while she recovers. One night, when Brian knows that Brian is watching, he steps into the pool naked. Brian feels uncomfortable and leaves. Angela returns to work at the hospital part-time to give her more meaning, and Katie watches Cora. Katie uses her free time around the house to search Angela and Brian's belongings, envious of their family. Angela comes home one day after work and catches Katie having sex at the guest house with which she keeps Brian to herself but is actually a woman, later revealed as Linda. After the initial embarrassment, Angela relaxes a bit and trusts Katie more without believing that she would ever be interested in Brian. Linda excitedly tells Katie that Angela will ask Linda to be her surrogate mother, but Katie can hardly hide how upset she is.

Apparently Angela still has an embryo from the same donor that conceived Cora, and she wants to use that so that the two can be full siblings. Katie confides in Linda that she is the donor and that her daughter Maddie was also hers but was born to a mother in Maine. Katie kidnapped Maddie when she was a baby because "the mother didn't take care of her" and now she worries that Cora is not getting enough attention and wants to take her. She drowns Linda in a lake and a couple later find her body. Four months later, Katie is carrying Angela and Brian's baby (and unknown to them, her own baby). Donna asks Angela what it means to Katie. Angela says Katie is paid forty thousand dollars and she doesn't expect anything else. In a flashback it is revealed that Katie, whose real name is Tara, can no longer have children of her own. As Katie's behavior gets stranger and delusional - Angela catches Katie lying to a mother in a park about both girls being her twin daughters - Angela grows more suspicious. Brian says everything will be fine when the baby arrives, after which Katie will get on with her life.

A few months later, Angela and Brian tell Katie they are getting a full-time nanny and Katie angrily accuses them of having a total stranger raise their child. Brian defends her, saying she just has different beliefs than her. It becomes clear that Angela and Brian are no longer so happy. Angela suspects that Katie is planning to keep the baby and disappear, and accuses her of killing Linda because she wanted it for herself. Katie fakes early labor. Brian is angry with Angela, believing she is taking medication and is going mad. The next morning, while Brian is at work and Katie is sleeping in the girl's bed in the main house, Angela sneaks into the guest house to sniff and uncover Katie's secrets. This confirms that she is mentally ill and was a previous donor. Angela gets a DNA test for the girls but finds Katie drugged her and rushes home to confront Katie. Katie admits everything and stabs herself in the stomach, which may harm the baby. Katie says she'll tell everyone that Angela tried to kill her.

Katie is taken to the emergency room and an emergency caesarean section is performed. A healthy boy named Gabriel is born. However, it is later revealed that the DNA test came back and showed that Maddie and Cora were related to each other and to Katie, meaning Angela was telling the truth. It is also revealed that she killed Maddie's parents before abducting them and claiming them as her own. In the end, Katie is locked up and Angela, Brian, Maddie, Cora and Baby Gabriel are now a happy family.
