The Final Inch shows the hard work of the often voluntary helpers in administering the polio vaccine in Afghanistan and India. Cases of polio are not uncommon in either country, although the vaccine has existed since the 1950s and has been administered orally since 1961. The film shows the work of volunteers from Rotarians , and the umbrella organization of Rotary clubs has established a "National Immunization Day" in various countries. It is also discussed that Muslim children from poorly educated households are kept away from the vaccination because "the US vaccines cannot be trusted."

The Final Inch was filmed primarily in 2007 in slums in Afghanistan , Pakistan and India . The financing came from the charitable spin-off of the Google group - Google.org . Its director, the doctor and epidemiologist Larry Brilliant, commented on the film as follows:
"The most contaminated environments are the poorest environments in the world, therefore, it is not surprising that 'the final inch' will be that inch of humanity that we don't see, that happens behind the curtain of poverty, where people die quietly with no one to hear them. It is a test of our civilization - how we care for the most vulnerable amongst us, how we care for the people who we don't see. "
