Zafy completed a medical degree at the University of Montpellier . After his return to Madagascar, he became Minister of Social Affairs. After the Ratsiraka coup , he was urged to resign. In 1988 he founded the National Union for Development and Democracy. At a national conference of the opposition in 1990, Zafy was elected chairman.
In 1993, Zafy succeeded in surpassing incumbent Ratsiraka in the first multi-party elections for almost twenty years. However, Zafy could not solve Madagascar's economic problems and therefore resigned in September 1996 after a vote of no confidence (July). In 1997 he lost the election to Ratsiraka.
