He was an opponent of the Nieuw-Malthusianisme, a Dutch movement for birth control. In 1902, Kouwer took part in the founding of an association for young children without care , in the same year became a member of the Nederlandsche Bond voor Ziekenverpleging (German: Dutch Association for Health Care ), and in 1903 became a member of the Nederlandsche Maatschappii ter Bevordering der Geneeskunst (German: Dutch Society for Promotion of medicine ), 1899 member of the Provinciaal Utrechtsch Genootschap van Kunsten en Wetenschappen (German: Provençal Utrecht Society for the Arts and Sciences ) and 1923 member of Hollansche Maatschappij der Wetenschappen (German: Dutch Society of Sciences ) in Haarlem.
As a gynecologist for Queen Wilhelmina of the Netherlands , he assisted her in a miscarriage that occurred in May 1902 and gave birth to her daughter Juliana on April 30, 1909 .
