McCarthy began his career with actions in which gravity was used as a metaphorical vehicle. The work includes drawings, sculptures, actions and performances, performance videos, films, and installations.

From 1974 onwards, his works became more aggressive and sexually provocative: The performances deal with brutality and self-destruction.

In the 1980s and 1990s, the performances (in the then international trend) were characterized by ironic distance, for example Painter .

In another form of work, large backdrops are set up in which actors play scenes as they please , which are recorded as videos and photographs. After the completion of such performances, the scenery, video material and photos count as art objects. McCarthy's son Damon was often involved in this work.

In 2008, an enlarged sculpture of Santa Claus was installed on Rotterdam's Eendrachtsplein . It is supposed to represent a Santa Claus who, instead of a Christmas tree, is holding a sculpture that makes some observers think of a butt plug .

In November 2009 he opened an exhibition in New York entitled White Snow , which was thematically based on the Disney adaptation of the German classic fairy tale Snow White and the Seven Dwarfs .

On October 16, 2014, he had a huge green inflatable sculpture, which he characterized as a "tree" but which has the shape of a butt plug, installed on Place Vendôme in Paris. He was slapped by a passer-by while visiting them.

McCarthy's works often have a thematic reference to American culture. They are in great demand on the international art market. McCarthy is one of the most influential contemporary artists.
