The building on a cruciform floor plan, which is 70.5 meters long and 37 meters wide, was built entirely with stones from the region, the altars are made of marble from Estremoz, Pero Pinheiro and Fatima. In front of the Basilica of the Rosary, a tribune with an altar, ambo and benches for concelebrants was set up in the large pilgrim square.

The bell tower is 65 meters high and is crowned by a bronze crown weighing 7000 kilograms, made in the Bolhão foundry in Porto, topped by a cross lit at night. The carillon consists of 62 bells that were cast and tempered in Fátima by José Gonçalves Coutinho from Braga. The largest bell weighs 3000 kilograms. The clock was made by Bento Rodrigues from Braga.

The marble angels on the facade were created by Albano França. A statue of the Immaculate Heart of Mary weighing 13 tons and 4.73 meters high was donated by American Catholics in 1958 . At the entrance to the basilica, above the portal, there is a mosaic depicting the Holy Trinity crowning the Virgin Mary. The three shepherd children were later buried in the church. The grave of Francisco Marto is in the right transept, the graves of Lúcia dos Santos and Jacinta Marto are in the left transept.

The 14 side altars each represent a mystery of the rosary , the golden bas-reliefs there are by the sculptor Martinho de Brito. The 15th secret is depicted as a relief by Maximiano Alves in the dome above the main altar. The cross vault is decorated with a surrounding mosaic, which shows the invocation of Regina Sacratissimi Rosarii Fatimae or pro nobis .
