Five female students have to pass a special test of courage to be accepted into a popular marriage. The motto is Truth or Scare and the rules are simple. The exam takes place at Vanguard Sanatorium , which is considered to be one of the scariest places in the world. Each girl is abandoned on a different floor and they have to endure five hours a night. In the tuberculosis ward of the former hospital, more than 63,000 people are said to have died of the white plague since 1910.

After breaking free of their masks, the frightened young women who hear commentary over the public address system first explore their surroundings, which are made up of dilapidated furnishings, disgusting objects and shocking photos. They try to find their teammates in order to fight the fear together, but instead are confronted with the ghosts of the dead. The situation turns dramatic when Heather finds the first dead.

Elizabeth is covered in ice in the hallway and is frozen to death. A short time later, Ashley arms herself with a shard of glass to kill her enemies, and accidentally catches Devon, who dies from the injuries. Richie, who has meanwhile rushed into the building from his surveillance screens, meets Ashley with Heather. Ashley suspects Heather, who found a chain with the initials RJV on the corpses, to have murdered the other two girls. But Richie admits that his grandfather Richard James Vanguard was the head of this sanatorium at the time. When Ashley tries to run away angrily, she runs into a power cable and dies too.

In the archive, Heather and Richie track down the history of the sanatorium. After an involuntary stay in the morgue, Heather and Richie flee through the death tunnel. Heather realizes that the initials of the five students form the word DEATH (death). At the end of the tunnel she reveals the truth to Richie: "I died here."

The former Waverly Hills Sanatorium served as the backdrop for the Vanguard Sanatorium . Erected in 1910 in Louisville , Kentucky , the building served as a special facility for treating tuberculosis patients. Jefferson County had one of the highest tuberculosis rates in the country at the time, which resulted in a gradual expansion of the sanatorium. Because it was feared that the sight of hearses could have a bad effect on the morale of the patients, deceased patients were transported away through a steam tunnel. In 1961 the sanatorium was closed due to the now low number of patients and reopened as a private clinic a year later.
