Princess Fantaghirò IV is the fourth film in the Fantaghirò series. As in the first three parts, Lamberto Bava took over the direction and Alessandra Martines took on the leading role in the title. The film was shown on television as a two-part series.

In the film itself, only a few characters from the first three parts appear, with the most striking fact that Kim Rossi Stuart, who plays the role of the hero Romualdo in the first three parts, does not appear in any new scenes. To get around this, his character has been adjusted so that another actor can take on the role. In addition, footage from the first part was used for the end of the film.

A black cloud passes through all kingdoms and kills man and cattle. Buildings also disappear without a trace. From the middle of the black cloud, gloomy figures appear, the three black knights, called famine, pain and disease. The young Parseltongue, prince of the island of Targo, faces up to the devils because the black wind has robbed his castle, his people and his family. But the devils turn out to be invincible and want to get rid of the brave youth. But before they can do this, Fantaghirò rushes to his aid, who selflessly followed his calls for help. A strange lightning bolt appears and the devils ride away unassailable. Parseltongue is saved, but still doesn't have his lock back. Only the advice of the black knights that they were created from the root of all evil. If you know his name, you will also know where the merciless demon who sent the devils and the black cloud is hiding. Parseltongue, on the other hand, taps the king of the blue mountains, Tohor, from whose direction the unlucky wind came and found its origin. Fantaghirò, however, would like to take Parseltongue with them to their castle, but shortly before they arrive there, the black cloud also reaches Fantaghirò's castle and kidnaps everyone in the building.

Meanwhile, the headstrong Princess Angelica, daughter of King Tohor, is on the lookout for her great love, who is known everywhere as the spring devil. In fact, the king's daughter finds her dream husband, who is captured by her father's soldiers. In the castle of the kingdom of the people with the little eyes, however, the source devil is exposed by the king's advisor for who he is; Tarabas, the lord of all evil. He should be held responsible for the evil that the black cloud brought with it. But Tarabas is not to blame for what he is accused of, he has turned away from his black power for ever and invokes his innocence. The king makes the decision to let a duel take place. It should be the ultimate fight; Tarabas versus Rufus. Rufus is the glorious son of the king, whom no one has seen yet and yet everyone knew of his existence. Rufus has lived in a dungeon in the castle tower since he was born. His mother died giving birth because Rufus was no ordinary child. He is half human, half animal. The legs and hooves of a horse carry the torso of a man and that of a man, who is distinguished from the other by two horns. Since Tarabas does not want to use his magic powers against Rufus in a duel, he has no chance against his incredibly strong opponent, who has always been treated by everyone like a worthless head of cattle.

In the meantime, however, Fantaghirò arrives at the castle with Parseltongue and saves Tarabas by speaking well to Rufus and treating him as the first one who is Rufus - a person. Together with Tarabas, Fantaghirò and Parseltongue flee the castle, which is in turmoil, in order to finally reach their destination and get their locks back. On the way there they cross the black forest and ride past the former kingdom of Tarabas. They are seen by the gnomes who immediately report the event to their new ruler, who is none other than the black witch.

But the witch has lost her cruelty and control over her magic powers, because she helped Fantaghirò to find Tarabas and thus to awaken Romualdo again. The gnomes want to use this chance to overthrow their ruler, as they had already done with Xellesia. But the black witch managed to escape and holed up in the room where Tarabas' mother found her end. Her face suddenly appears from the fountain into which Xellesia was pushed by the gnomes and Xellesia urges the black witch to revive her with an elixir of life. She has no other option and frees her new accomplice from her prison. Xellesia immediately worries about her son and learns from her deliverer what has recently happened on earth. Her worst fears are confirmed and so Tarabas' mother, worried about her son, immediately sets off with the black witch on a flying broom to save him.

At the same time the three kings arrive at their destination, the Nekrad volcano. With the help of Parsel's riddle beetles, which showed you the way to get there, you will also find out the name of the villain to be confronted. His name is that of the volcano, only read backwards - Darken.

Finally they get inside the volcano, where they are surprised by a servant of Darken. The slave Fidor brings the three to their final destination, the ruler's throne room. However, this is nowhere to be found. Only when Parseltongue sits on the villain's throne do Tarabas and Fantaghirò understand what that means. Parseltongue is the one they were looking for all along. But Parseltongue was only used. The strange lightning bolt that appeared when Fantaghirò saved Parseltongue from the three black devils was Darken, who drove into Parsele's body to make absolutely sure that the three really arrive in his realm. When Darken appears after stepping out of Parsel's body, the young warrior is lying unconscious on the ground. Now the demon Fantaghirò and Tarabas reveals his motives; he wants Tarabas to become the dreaded prince of evil again. If he does not do this, Fantaghirò will lose her life. But Tarabas misjudges the seriousness of the situation and objects. At this moment Darken pulls out a dagger and throws it through Fantaghirò's chest. The brave queen dies in Tarabas' arms and it all seems to be over for good.

But Tarabas wants Darken Fantaghirò to come back to life. Therefore, he even promises him that he wants to become what he was once called to be again. Believing that he has achieved his goal, Darken brings Fantaghirò back to life. At the same time, Angelika and her brother Rufus arrive in the volcano, as shortly after Fantaghirò and her companions had escaped from their father's castle to travel after Tarabas and Fantaghirò. A big mistake, as it turns out, because Tarabas continues to refuse to get angry again. Darken gives Rufus a dagger to kill Fantaghirò, Tarabas in love is supposed to prevent this by stabbing Rufus. But Tarabas does not want to be guilty of this crime. Rufus doesn't want to harm his love either and uses the dagger to free her from her bonds shortly after Darken has made his promise true and transformed Rufus into a real person. Rufus, however, accidentally jumps into the boiling water of the volcano, dies in agony and is drowned in the hot water. Now Darken sees only one chance and faces a duel with Tarabas, in which a terrible secret is revealed. Darken, the origin of all evil, is the father of Tarabas. Darken kills himself, making it look like Tarabas is his killer. At that moment, Xellesia and the black witch also reach the inside of the volcano, after the black witch in particular had some problems with the broom flight. Tarabas is reconciled with his mother and Darken's corpse disappears without a trace - his death, a trick, as everyone now knows.

During the duel, Fidor frees Fantaghirò from her prison shortly after Darken shows his servant who he really is - Romualdo. But Fidor does not reveal himself and instead saves the little Parseltongue with Fantaghirò and its two castles, which have been reduced in miniature.

The volcano trembles, it begins to erupt. Only Xellesia knows the only way out; it leads through a huge, eerie mouth, with three rows of pointed teeth that open and close faster and faster from row to row. In order to block this mechanism, Xellesia sacrifices herself and allows her teeth to pierce so that the others can travel on unhindered, because they only have three days to bring their locks into their realm and thus save them.

Parseltongue says goodbye to his friends, as his path is considerably longer, and gives Fantaghirò his puzzle beetles as a goodbye. Darken, however, continues to intrigue and hides in the body of the black witch and urges Fantaghirò to kiss Tarabas. As if hypnotized, she does this and Tarabas transforms into the human-devouring monster again. But before he can kill his beloved, he is hit by an arrow from Fidor's bow. Angelica had freed herself and Fidor because the community had previously been captured by King Tohor's army. Darken reappears for a moment and reveals to Fantaghirò who Fidor really is. The latter flees because he is ashamed of his disfigured face and falls into what appears to be death while fleeing from Fantaghirò. Fantaghirò loses her courage, but still wants to save her people and her family. So she goes to the final fight against Darken in the ruins of her castle.

The sun is about to set for the third time, you don't have much time, but you can defeat one of the black devils. But there were three in total. Unexpectedly, Tarabas rushes to her aid and also destroys the second black knight. But the third is still ahead of the two heroes. They can only destroy Darken with his own sword. When Tarabas can finally take the weapon, he is stopped by Fantaghirò. He should not be guilty of the crime of killing his father. Darken finally realizes that he can never force Tarabas to become the dreaded Prince of Evil again and wants to put an end to him by killing Fantaghirò and his own son with Tarabas' sword. But Fidor did not perish in the fall on his escape. He lives and stabs Darken with the black sword of the cruel demon. The curses disappear and the castle is transformed back, only Romualdo remains in the form of Fidor.

While Tarabas goes back to learn to reciprocate Angelika's love in order to make her happy, Romualdo again flees in agony from his lover, but she still does not give up her love and assures him that she will always love him, no matter what form he is is trapped.

After the oath, he emerges from his hiding place and Fantaghirò realizes that her lover has turned back again. Both kiss deeply and are happy to have survived all the dangers.
