Electronic health professional card

The electronic health professional card (eHBA, English Health Professional Card , HPC) is a personal chip card in credit card format for members of the health professions in the German health care system.

In addition to its function as a visual ID, it offers cryptographic functionality for authentication , encryption and qualified electronic signature .

The legal basis for the electronic health professional ID was created with the new § 291a SGB ​​V on January 1, 2004 by Art. 1 of the GKV Modernization Act (GMG).

According to Section 291a Paragraph 5 Clause 5 and Paragraph 5a Clause 1 SGB V, health professionals need the electronic health professional ID card to access the data of the applications of the electronic health card according to Section 291a Paragraph 2 and 3 SGB V. The technical requirement is the connection to the medical institution to the telematics infrastructure of the German healthcare system. Access to the data of the applications of the electronic health card is alternatively possible for employees of a medical institution authorized by a health professional via an electronic institution ID ( Security Module Card Type B (SMC-B) ). Healthcare professionals themselves can also use the SMC-B for access.

The ID cards are technically specified by the company gematik .

Electronic health professional card

Similar to the electronic health card, the new plastic health professional card contains a microchip that offers authentication (electronic identity verification), encryption and electronic signature. Depending on the medical profession, the electronic medical professional ID will also have a visual ID function. Technically identical, but different in terms of personalization and external design, at least the following variants must be distinguished:

Electronic pharmacist ID

The electronic pharmacist's ID is issued by the respective regional chamber of pharmacists at the request of the pharmacist . The chambers can commission external service providers for the technical processing.

Electronic doctor's card

Similar to the electronic health card, the new plastic doctor's card contains a microchip that offers authentication (electronic identity verification), encryption and an electronic signature. With the help of the electronic doctor's ID, doctors will in future be able to access the patient data on the electronic health card, provided the patient has released it. Since the electronic doctor's ID is provided with a qualified certificate , electronic documents can be provided with a qualified electronic signature , i.e. legally digitally signed . The electronic doctor's card makes telematics applications such as electronic prescriptions, electronic drug documentation and electronic doctor's letters possible. Due to the external design, which was protected by law, this ID will also replace the doctor's previous ID. All licensed doctors receive the electronic doctor's ID upon request . The electronic medical card is issued by the state medical associations .

It is "the acquisition of an electronic health professional card (eHBA) - also known as a doctor or psychotherapist card - optional." Accordingly, the health professional card is the generic term , the doctor's card would be one of the sub-terms . However, the medical association's new doctor's ID card will only become an electronic doctor's ID after a complex and expensive upgrade . So the terms doctor's ID and health professional ID are not synonyms for several reasons .

An outwardly valid old electronic doctor 's ID ("precursor card", light ID) can be distinguished from the new electronic medical professional ID for doctors by the various designs of the chip card module . It remains to be seen whether conversion is even possible. A complex ordering process is described with an identification of the applicant according to the Post-Ident procedure for the new "high-quality ID card" with a mandatory activation within 28 days . In December 2019, the Chaos Computer Club found a potential for abuse ("data leak", "security gap ", "weak point") when the chip card was delivered to a desired address . In the official texts, the term “ doctor's card” printed in five languages ​​is not used. It also remains open whether the state chambers of psychotherapists issue such medical IDs for non-doctors.

There are two different electronic medical ID cards, as the Westphalia-Lippe Medical Association informed in August 2020. Once the "electronic health professional ID (eHBA) as a doctor's ID" for the applications of the telematics infrastructure (TI). In addition, you can apply for a so-called “electronic doctor's ID light (eA-light)” free of charge from the state medical associations as a replacement for the previous paper ID. It is also abbreviated EA-LIGHT; the English adjective light means 'light', 'mild', 'indulgent', 'tender'. This ID is not suitable for TI applications. The EA-light may be, for example of the sago authentication ( O nline -Use for A Ussen g utachter by S GB IX ) may be used. On the back of the eA-light, besides the signature of the owner, there is the ID number and the personal uniform training number (EFN) for the electronic recording of the CME points ( Continuing Medical Education ) at training events , but neither with the corresponding barcode and QR code the doctor number or the lifelong doctor number (LANR).

Both ID cards can be distinguished externally by the design of the contacts on the chip card module. The eHBA module has a square in the middle; the center of the eA-light module, on the other hand, is partly curved. In addition, in the case of the eA-light, the surname of the owner is given first and then his first name ; it is the other way around with eHBA. The five-year period of validity of the eA-light is specified to the day; the eHBA, on the other hand, only names the expiry month. At the moment you can only order the chargeable eHBA from two so-called trust service providers , T-Systems ( Deutsche Telekom AG ) and Bundesdruckerei . Generation G2 is now being sold , before that there was the forerunner generation G0 (not to be confused with the forerunner card of the light ID card), which can be exchanged for G2, and generation G1. In contrast to the health cards, there is no indication of the card generation on the health professional cards. The purchase price (with binding contract ) is made up of a one-time fee and an annual fee.

For security reasons, the doctor needs two different PINs and a PUK together with the eHBA . It is possible to have several valid electronic doctor's IDs at the same time, for example two or more chargeable eHBA and an additional free eA-light. This is the only way for contract doctors to identify themselves to patients or authorities as having a valid license to practice medicine or to purchase prescription drugs from pharmacists without a prescription without interrupting the practice process . The doctor is entitled to appropriate IDs even if he is a compulsory member of different medical associations. Both forms of identification lose their validity when the deadline expires or when the Medical Association is blocked after a license to practice medicine has been withdrawn. The eHBA does not lose its validity when the contractual relationship with the so-called approved provider is terminated .

Electronic dentist ID

The respective state dental associations are responsible for issuing the electronic dentist cards. Since the ID cards contain certificates and key material from the dental public key infrastructure (PKI), certification service providers must also be involved in the creation of the electronic ID cards. The state dental associations act as the registration authorities with regard to the PKI properties of the ID cards.

Electronic psychotherapist card

The electronic psychotherapist cards (ePtA) are issued by the local state chambers of psychotherapists. These ID cards can be used to sign the online billing with the statutory health insurance associations in a legally binding manner ("electronic signature"). They are also necessary in order to be able to access data on the new electronic health insurance card of patients, the electronic health card (eGK), or to be able to save data on it.

Electronic professional ID

The electronic professional ID card (eBA) in the healthcare sector is technically identical to the medical professional card and is described by the specification for the Health Professional Card. In contrast to the medical professional ID cards, the professional ID card is intended for health care personnel (e.g. MTA or PTA ). Another difference is that the professional ID card has restricted access to the health card and only those are sufficient to prepare for the activities of a health professional. A doctor's assistant can read out the protected insurance data, but cannot change the emergency data of the eGK. Only a doctor’s medical certificate is authorized for this.

In medical and dental practices as well as in pharmacies and hospitals, the other professional groups of these institutions should access the eGK via the security module card type B (SMC-B). Like the health professional card, the professional card is always required if a signature (electronically and legally secure) is to be provided. The Center for Telematics and Telemedicine (ZTG) is carrying out the pilot project "electronic health professional ID" by issuing the card to 1000 physiotherapists . The eBA is issued by the electronic health professions register .

costs

It is currently unclear who will bear the costs for the planned issue of the Health Professional Card by the respective chambers. The form of financing (fees, one-off payments, etc.) for software and hardware has not yet been determined either. Medical associations point out that their members have "running costs". In 2018, these are between € 7.90 and € 9.90 per month for paperless invoicing.
