Karl Ulrich Petry

Karl Ulrich Petry (born July 3, 1957 in Wetzlar ; † April 21, 2020 in Wolfsburg ) was a German gynecologist , professor and chief physician of the women's clinic at the Wolfsburg Clinic . Petry was known as a pioneer in HPV screening in Germany and for the surgical technique for the treatment of extensive precancerous stages and microinvasive carcinomas of the vagina. He was head of the HPV screening pilot project with the Deutsche BKK in the greater Wolfsburg area (WOLPHSCREEN) and head of the study on HPV epidemiology in young women in their early to mid-twenties (WOLVES).

Life

Petry studied human medicine at the Philipps University of Marburg from 1976 to 1983 . He was licensed as a doctor in 1983 and began his clinical training initially as an assistant doctor in surgery at the Remscheid Municipal Clinic. In 1985 he moved to the gynecological clinic in the deaconess hospital in Kassel as an assistant doctor and in 1986 he became a medical officer (department doctor) at the Sokoine Regional Hospital in Lindi , Tanzania. His focus there was surgery and gynecology. He began research into HIV , schistosomiasis and other infectious diseases. Petry became the spokesman for the development workers of the German Development Service (DED) in Tanzania. In 1989 he became an assistant doctor at the gynecological clinic of the Hannover Medical School (MHH), where from 1990 he headed the establishment of the dysplasia consultation hour and the research focus HPV in gynecological oncology. He worked on the role of HPV in the development of gynecological neoplasias , developed new therapeutic methods and researched the role of immunodeficiency . In 1992 he completed his doctorate as Dr. med. from. In 1995 he became a specialist in gynecology and obstetrics and in the same year a senior physician at the MHH women's clinic. The scientific focus was on the use of HPV testing to improve the prevention of cervical cancer (cervical cancer).

In 1999 Petry became the clinical director of the Hanover-Tübingen study on primary HPV testing in cancer prevention, which recruited more than 8000 women (from 2000 transferred to "Cervical Cancer Consortium Europe"). In 2000 Petry received the Venia legendi for "Gynecology and Obstetrics" ( habilitation ). Since 2003, Petry has been the chief physician of the women's clinic at the Wolfsburg Clinic with a focus on gynecological oncology with a certified cancer center and dysplasia unit as well as specializing in ultra-radical surgical procedures and plastic-reconstructive operations on vagina and pubic area. In 2005 Petry became APL professor at the Hannover Medical School.

Clinical and scientific contribution

Petrys' initial scientific focus was initially on the role of cellular immunodeficiency in the development of cervical cancer . By comparing the clinical course of HIV-infected, immunosuppressed women after organ transplantation and immunocompetent women, it was possible to prove that cellular immunodeficiency favors the persistence and also the malignant progression of HPV infections. This work, along with others, contributed to the classification of cervical cancer as an AIDS- defining disease. From 1995, investigations into the use of HPV detection for improved cancer prevention followed. With the Hanover-Tübingen study, which has been conducted jointly with Thomas Iftner since 1999 and which was brought into the Cervical Cancer Consortium Europe, it was possible for the first time to demonstrate in a prospective international muticenter long-term study that real cancer precursors (CIN3) can be detected significantly better with HPV testing than with conventional cytology. The first German HPV screening pilot project (Wolfsburg Primary HPV Screening project: WOLPHSCREEN) was founded in Wolfsburg in 2006. In cooperation with the Deutsche BKK and Audi BKK and 34 gynecological practices, more than 25,000 women in the age group over 30 were included by 2016. WOLPHSCREEN demonstrated better prevention of cervical cancer through earlier detection of CIN3 + in HPV screening, also showed the feasibility of HPV screening with 5 annual intervals in everyday German care, but also pointed to new challenges, such as the necessary optimization of colposcopy and improved triage of HPV positive women. WOLPHSCREEN anticipated many elements of the new cervical cancer screening decided by the Federal Joint Committee from 2020 and in 2019 demonstrated an improved detection of CIN3 + and the resulting better prevention of cervical cancer (Horn J et al., BJC 2019)

Since 2009, the clinic in Wolfsburg has been conducting an epidemiological study on HPV in young women in their early to mid-twenties under Petry's direction (Wolfsburg HPV epidemiological surveillance: WOLVES). WOLVES and WOLPHSCREEN thus provide a complete HPV epidemiology for women between 20 and 70 years of age for the Wolfsburg region. This shows that the crucial HPV infections that lead to CIN3 and carcinomas are acquired early in life, usually well before the age of 30. This results in the possibility of theoretically eradicating cervical cancer within a few decades by combining HPV vaccination and HPV screening.

Another focus of Petrys is the establishment of laser skinning colpectomy for organ-preserving therapy of extensive carcinoma in situ and micro-invasive carcinoma of the vagina. Only the superficial vagina is excised in one piece with a laser under colposcopic view. As a result, the function of the vagina is preserved and a full-fledged histological specimen is still available. Finally, Petry also devoted himself to the development of an independent Internet-based quality assurance in colposcopy.
