The Schleemer Bach rises in Hamburg-Rahlstedt in the small forest area of Hegen near Rahlstedt-Ost. It flows past the Wilhelmstift children's hospital and is parallel to Grunewaldstrasse and also the state border. Then it mostly flows in a southerly direction past the main cemetery Öjendorf and through the Öjendorfer See ; for this it is both an inflow and an outflow. The Jenfelder Bach and the Barsbek flow into the Schleemer Bach . At the border of Hamburg-Billstedt and -Billbrook near the Red Bridge, it joins the Bille after about 10 kilometers .
In the post-war period, the river partially degenerated into a sewer ditch. But this was later dismantled. In the Wilhelmstift area, a wetland area was removed that can be crossed over a wooden bridge.
Fishing in the Schleemer Bach is prohibited all year round, including the Schleemer pond.
