Felodipine is a drug from the group of calcium antagonists of the 1,4-dihydropyridine type and can greatly reduce blood pressure by expanding the smooth muscles of the vessels . Therapeutically, it is mainly used to treat arterial hypertension. The drug is used as a racemate of atropisomers .
Mechanism of action
Felodipine blocks the influx of calcium into the heart muscle cells, the smooth muscles and the excitation and conduction system of the heart. In the heart, this leads to reduced contractility (negative inotropic ) and reduced O 2 consumption. On the smooth muscles, it causes the small arteries ( arterioles ) to widen and thus reduce the afterload. At therapeutic doses, the effect of felodipine and other dihydropyridine calcium antagonists is hardly on the heart, but almost exclusively on the blood vessels.
Indications
Felodipine is approved in Germany for the treatment of essential arterial hypertension .
Side effects
Syncope , vomiting and arthralgia occur rarely . Restlessness, tinnitus , dyspnoea , constipation , photosensitivity, pollakiuria , menorrhagia and weight gain occur very rarely .
The consumption of fruits from the grapefruit family can lead to a delayed breakdown of felodipine and thus to an increased availability of active ingredients in the body.
Contraindications
The administration of felodipine is contraindicated in the following diseases: acute myocardial infarction (within the first 8 weeks), decompensated heart failure , hypertrophic obstructive cardiomyopathy , high-grade aortic valve stenosis, unstable angina pectoris, 2nd and 3rd degree AV block , stroke within the last six months , severe renal insufficiency .
