He moved to the Albert-Ludwigs-University of Freiburg and was there in 1889 for Dr. med. PhD. In Freiburg im Breisgau he found a job as an assistant at the anatomical institute. He later set up as a practical country doctor in the Black Forest. After he had pursued further studies in dentistry in 1898, he completed his habilitation in 1899 at the Kaiser-Wilhelms-Universität Strasbourg for this subject. In 1906 the private lecturer became an associate professor there .

In the same year, Römer went to the University of Leipzig in the same position , where he was director of the Dental Institute and in 1920 he became professor for the pathology of dental diseases in Leipzig . He participated in organizational tasks at the university and was dean of the medical faculty in 1925/26 . For the academic year 1928/29 he became rector . In 1906, Römer was made aware of Rudolf Steiner's anthroposophical teaching by his Swiss colleague Alfred Gysi . At least since a personal meeting with Steiner in 1910, Römer was strongly influenced by anthroposophy. Testimony of this gives u. a. his work on dental caries with reference to the results of spiritual research by Dr. Rudolf Steiner's (1921). After he retired in 1934 , he withdrew into private life. He died at the age of 86 in his country house in Berchtesgaden as a man with an international reputation in dentistry.
