The sewer worker Arthur is traveling with his new employee George in the sewers. George, who was convicted of drug trafficking, was given this work on probation . Behind a crumbling wall, Arthur discovers a tunnel that he does not know yet and wants to examine it. George, who cannot swim, does not want to follow him. At the end of the tunnel, Arthur is killed by an unknown figure.

Kate works as a German agent for a model agency in London . Guy harasses her at a posh party, but manages to ward off his advances. Now she wants to go to another party to meet George Clooney . She decides to take the London Underground and goes to Charing Cross station . Since she has no change for the machine, she buys a ticket from a homeless person for £ 20. As she waits on the platform, she drinks a bottle of vodka and falls asleep.

When she wakes up again, the stop is completely empty. She runs upstairs seeking help, but there is no one there and all exits are closed. Then another train comes and Kate gets on. However, the train stops after a few minutes in the middle of a dark tunnel. Kate goes up to speak to the driver but can't find any. Instead, Guy suddenly approaches from behind. The young man is drugged and tries to rape her . But before that happens, a door suddenly opens and Guy is pulled away. A few moments later it reappears briefly, covered in blood, but is pulled away again. Kate takes flight.

On the way she meets a dog who leads her to the hiding place of the homeless couple Jimmy and Mandy. Kate recognizes the girl again, she had sold her the ticket. Kate can finally persuade the boy with 50 pounds to go with her to the night watchman. On the platform they see Guy crawling along the platform, seriously injured. Jimmy wants him to die, but Kate pays Jimmy another £ 50 to help him. He pulls Guy onto the platform. While Jimmy is running back to his hiding place because their dog, which should have stayed with Mandy, suddenly appears behind him covered in blood, Kate makes contact with a night watchman via an intercom. He doesn't believe her story and demands that Kate drag the injured guy into the surveillance camera’s field of view. Meanwhile, Guy has died and Kate is now pulling the lifeless body into the field of view of the camera. The night watchman sees him and is murdered a short moment later, before he can get help.

Jimmy sees a trail of blood in front of his hiding place and cannot find Mandy. Together with Kate he wants to go through the tunnel to the next stop when suddenly a train rushes up from behind. The two can escape into a side shaft. The train stops. The two briefly argue about whether they should take the train or escape. Jimmy chooses the former. On the train he is pulled to the ceiling by the murderer and brutally killed. Kate runs away again. She crawls through a sewer shaft where she finds the bloated corpse of the sewer worker Arthur, before reaching a warehouse via a ladder. She is walking through the storeroom when suddenly the light goes out; she takes refuge in a corner. In the light of the torch she looks into the face of a gruesome human creature.

In the next shot you can see that she is trapped in a cage in the sewer system. The murderer, dressed only in shorts and slippers, with the misshapen body and a stiff gait, gathered several victims there and maltreated them with a harpoon . Among the victims is George, the drug dealer from the first scene. Kate realizes that the bars don't go down to the floor and tells George to dive to an open cage; however, George cannot swim, so Kate does the job herself and frees herself and George. She catches the creature's eye with a high-heeled shoe, but this doesn't prevent it from further activities. In the abandoned tunnels they discover a room that looks like an operating theater ; old medical equipment is everywhere. There they find Mandy on a gynecologist's operating chair. George thinks her dead, but after they leave the room, the audience sees that Mandy is still alive before the killer finally arrives. He hears Mandy's voice, which is injured and tied to the operating chair, and goes to her. After the latter has prepared as if for an operation while she begs for mercy, he stabs her in the abdomen and saws her up. Now you can see a row of jars with embryos and a picture of an older doctor with a little boy who bears a resemblance to the creature and has the same greenish skin color.

Kate and George keep looking for an exit until they come to a brightly lit room where they find the dog in a bird cage. George freaks out in desperation and smashes the windows and a table. However, the two are tracked down by the killer and there is a fight between George and the killer. Traumatized Kate has to watch as the murderer kills George by ramming his head against a sharp metal piece that got stuck on the wall when George kicked the table away.

Kate is pursued by the killer through catacomb-like corridors, in which she loses her orientation, but she finally manages to overpower the perpetrator who takes her pleasure. She rams a crowbar lying next to her, which is hanging on an iron chain, into his neck, pulls out the anchorage of the iron chain on a wall near rails and wants to throw it onto a conductor rail of the subway in order to kill him . The murderer gets up and walks towards Kate with the iron bar stuck in his throat. Before he succeeds in killing her with a block, a train drives over the chain that is stuck on the tracks and the force that arises tears the crowbar through the murderer's throat, who then bleeds to death.

In the final scene, Kate sits with the dog on the platform at Charing Cross station, where the story began. Since she now looks like a homeless person herself, dirty and in broken clothes, one of the first passengers places a coin on the ground in front of her.
