Federal popular initiative for strong care (care initiative)

The Federal Popular Initiative for Strong Nursing (Nursing Initiative ) is a Swiss popular initiative of the Swiss Professional Association of Nursing Women and Nursing Specialists SBK . It demands that the Confederation and the cantons are obliged to ensure adequate nursing care for the needs of the Swiss population. This is to be done in particular through investments in the training of sufficient nursing staff, an improvement in working conditions and partial autonomy in the settlement of nursing services with the health insurance companies.

According to calculations by the Swiss Federal Statistical Office , the number of people over 65 in Switzerland will rise to 2.7 million over the next 30 years. At the end of 2014 it was 1.5 million. This means that the proportion of old, chronically and multiply ill people will continue to increase sharply. In addition, the length of stay in hospital after surgical interventions - and sometimes also in the event of illness or accident - is steadily decreasing. One of the consequences of this is that more patients need to be cared for, instructed and advised in less time.

Switzerland does not succeed in training sufficient staff for the existing and future needs for professional care. The situation is particularly acute among qualified nurses . For example, the number of training in 2014 across all training levels ( qualified nursing professionals HF and FH , specialist women / specialists in health FaGe EFZ ; assistants in health and social affairs EBA ) was only around 56 percent of the calculated requirement, and only 43.1 percent of the qualified nursing professionals trained at tertiary level Percent. The degree of fulfillment in the following years was also well below the requirement of 6,075 nursing diplomas at the tertiary level: 2,580 in 2015, 2503 in 2016 and 2,632 in 2017.

The shortage of nursing staff is exacerbated by the high proportion of people leaving their jobs: 45.9 percent leave their jobs early.

Initiative text

The Federal Constitution1 will be amended as follows:

Art. 117c2 Care

1 The Confederation and the cantons recognize and promote long-term care as an important part of health care and ensure adequate, high-quality care that is accessible to all.

2 They ensure that a sufficient number of qualified nursing professionals is available to meet the increasing demand and that those working in nursing are deployed according to their training and skills.

12. Transitional provision to Art. 117c (care)

1 Within the scope of its responsibilities, the Confederation shall issue implementing provisions on:

a. the definition of the care services that are provided by nurses at the expense of the social insurance:

1. under own responsibility,

2. on medical order;

b. the appropriate remuneration for care services;

c. working conditions that meet the requirements for those working in care;

d. Opportunities for professional development of those working in care.

2 The Federal Assembly shall adopt the statutory implementing provisions within four years of the adoption of Article 117c by the people and the cantons. Until the statutory implementation provisions come into force, the Federal Council shall take effective measures to remedy the shortage of qualified nursing staff within eighteen months of the acceptance of Article 117c by the people and the cantons.

The care initiative was launched on January 17, 2017. The initiative was submitted on November 17, 2017. The Federal Chancellery confirmed on November 29th that the initiative came about with over 114,078 signatures.

On March 9, 2018, the Federal Council announced that it would reject the care initiative without a counter-proposal. On November 7, 2018, he specified his rejection in his message to Parliament. The main reason was that the competency required for billing certain care services would lead to additional costs in the compulsory health insurance and to increased premiums.

Indirect counter-draft

The Commission for Social Security and Health of the National Council (SGK-N) decided on January 24, 2019 to oppose the care initiative with an indirect counter-draft in the form of a parliamentary initiative (19,401 for strengthening care - for more patient safety and more care quality).
