The Prva Liga RS 2018/19 was the total of the 24th season of the Republika Srpska and the 17th as only the second highest division in Bosnia-Herzegovina in men's football . It started on August 11, 2018 and ended on June 8, 2019.

The champions became the FK Borac Banja Luka , who managed to immediately rise again.

To the remaining nine teams from the previous year the relegated FK Borac Banja Luka from the Premijer Liga and the two promoted FK Alfa Modriča and OFK Sloga Gornje Crnjelovo from the Druga Liga were added.

As in the previous year, the season was carried out in a two-stage format. Initially, the twelve teams played against each other twice on 22 match days, divided into a first half and a second half. Then the best six clubs qualified for the promotion round in which the promoted team was determined. The bottom six teams identified three relegated teams in the relegation round. The results from the preliminary round were transferred to the promotion and relegation round.
