The Río Mizque begins at the confluence of the two rivers Río Wila Wila and Río Cueva Pampa in the village of Vila Vila on the southwestern edge of the Bolivian Cordillera Oriental at an altitude of 2512  m . From there, the Río Mizque flows first in a south-easterly, then northeast and later again in a south-easterly direction and flows into the Río Grande in the "triangle" of the three departments of Cochabamba , Chuquisaca and Santa Cruz after 291 km .

On its course in the department of Cochabamba, the river flows through the municipality Vila Vila and the municipality Mizque in the province Mizque , as well as the municipality Aiquile , the municipality Omereque and the municipality Pasorapa in the province Narciso Campero . On the last section, it also forms the border with the Municipio Moro Moro , the Municipio Vallegrande and the Municipio Pucará in the Vallegrande province of the Santa Cruz department.

The Río Mizque is not navigable along its entire length and, depending on the season, is subject to considerable water fluctuations.
