On Valentine's Day, the small, devout mining town of Harmony is struck by a tragedy. An explosion occurs in the nearby mine, triggered by the carelessness of Tom Hanniger, the mine owner's son. The pals are buried for a long time. After the rescue, it becomes clear that miner Harry Warden killed everyone else with a pickaxe in order to have the remaining air for himself. He can be rescued, seriously injured, but is in a coma.

The plot of the film begins exactly one year later: Harry Warden wakes up from his coma, the images of what is happening in the mine are still in my memory. With brutal violence he wages a massacre in the hospital, killing numerous people, tearing out the hearts of some and painting hearts on walls and mirrors with the blood of his victims. He then leaves the hospital and returns to his old mine where he is there is a Valentine's Day party among teenagers. Among the guests are Tom Hanniger, Sarah, Axel and Irene. Axel and Irene are a couple and are waiting for Tom and Sarah, who are also together. Together they want to enter the party in the gallery, but Tom forgets something and walks back to the car. When Sarah goes into the tunnel a little later, she is horrified to discover that the ground is littered with cruelly carved corpses. The murderer Harry Warden, in miner's clothing with a gas mask and headlamp and armed with a pickaxe, rages among the youth. After a short time the police arrive and an officer shoots the gunman who was about to kill Tom and then disappears into the mine. The four young people survive the massacre.

Ten years later - on Valentine's Day - Tom, who has still not overcome the cruel deed, surprisingly returns to town. Tom plans to sell the mine he inherited from his recently deceased father, which is met with significant opposition in the city. After the bloodbath in the mine he disappeared without a trace and broke Sarah's heart, who has meanwhile married Axel. Axel - now the sheriff of Harmony - has an affair with Megan, who is also pregnant by him. As soon as Tom has returned, the nightmare begins again: Little by little, numerous residents of Harmony are executed by the masked murderer, with the pickaxe always serving as a murder weapon. It hits, among other things, the police officers who put Harry Warden in the mine ten years earlier, as well as Axel's affair Megan. Sarah barely escapes the murderer in a supermarket. The residents of Harmony consider Harry Warden, who was believed to be dead, to be the killer and blame Tom for his new homicide train. However, it turns out that Harry cannot be the killer, as he was lynched by the police ten years ago. Axel - who suspects Tom and is afraid that Tom will take his wife off his feet - and Tom accuse each other of committing the murders in Harry's tradition. Finally there is a showdown in the mine when Sarah has to decide which of the two she thinks is the killer. His face was covered by the mask in all murders, so that nobody knows who is actually in the miner's clothes.

It becomes clear that Tom is the killer when he suddenly imagines he's seeing Harry Warden and tells Sarah to shoot him. In a flashback, we learn that Tom spent seven years in a psychiatric hospital, as Harry Warden has lived on in his body since that moment in the mine. Tom slips into the role of Harry Wardens again and again and kills with his clothes and the pickaxe without being aware of it. He attacks Axel, who was right from the start with his suspicions about Tom, and there is an explosion in the mine, which Tom buried.

When the rescue workers want to rescue the apparently lifeless Tom, he can kill the rescuer with the last of his strength, steal his equipment and run away from the mine. While Axel assures the authorities that Tom died in the mine, he can be seen hobbling out of the mine and disappearing.
