Epworth is in the southwest center of Dubuque County. The Mississippi , which forms the border with Illinois and Wisconsin , is about 25 km to the east. Neighboring towns are Bankston (9.8 km north), Graf (10.2 km northeast), Peosta (8.3 km east), Cascade (21 km southwest) and Farley (6.9 km west).

US Highway 20 , which runs from Waterloo to Dubuque, runs along the southern outskirts of Epworth in a west-east direction . The old Highway 20, which runs parallel to it, runs through the city center. All other roads are county roads or subordinate inner-city connecting roads.
A Canadian National Railway runs parallel to the old Highway 20 through the center .
