The film reproduces three episodes from the life of the main actor Nanni Moretti, which he wrote down in his diary.
- On the Vespa , plays in Rome during the summer . The city seems very deserted because many residents are on vacation. To Moretti's regret, most of the cinemas are closed or only show sex films, cheap horror flicks or Italian films. Going to the cinema becomes a torture in summer. Moretti prefers driving his Vespa through Rome and looking at the houses. His favorite neighborhood is Garbatella, a district in the south of Rome that was built between 1920 and 1929.
- The islands : Moretti, who needs peace and quiet to work on a new film project, would like to visit a friend in Lipari . But the hoped-for peace is missing because the island suffocates in traffic. So the two continue to Salina , then to Stromboli , Panarea and finally to Alicudi , where there are few people, no roads and no electricity. Here they find peace, too much peace.

As in most of Nanni Moretti's films, Caro Diario also has a mix of fictional and autobiographical elements. The film is always based on comical situations that Moretti experiences or brings about himself. In addition, the first part of this film is also a homage to Rome, although nothing of the city's celebrities can be seen and the ugliness of Rome is being shown, for example with the new building districts or on the beach, in search of the location the Pier Paolo Pasolini was murdered.
