The OSE operated local traffic around Athens until the late 1990s . After the success of the other local train companies in the greater Athens area (ISAP and Attiko Metro, the Athens Metro and Tram AE, the tram), it was decided to develop a similar model for its own local transport. The OSE subsidiary Proastiakos AE started operations with the opening of the new rail route to Athens Airport. After fundamental renovations, all local transport routes were gradually transferred to the new company. In 2007, the “Proastiakos Thessaloniki” opened a second route network. In July 2007, PROASTIAKOS SA was integrated into TRAINOSE SA, the nationwide rail operator.

The first line was still planned as an OSE line and built from 1994, the bridges were completed in 1997. For reasons of synergy, the route runs parallel to the Attiki Odos motorway , which was built at the same time. In 2004 the airport line went into operation. Vehicles were procured from the Swiss Stadler Rail and Siemens . On September 27, 2005, the first section to Patras was opened. The Proastiakos now travels to Kiato and touches the route of the old meter-gauge Peloponnese railway in places (now without traffic). This normal-gauge line is to reach the port city of Patras in the future.
The meter-gauge suburban train runs every hour between Agios Andreas and Rio; the extension to Agios Vasilios was discontinued in May 2018. A bus service operated by the operating company shuttles between the Kastellokampos station and the University of Patras , with a stop at the University Hospital Patras .

Criticism and competition
The low capacity utilization of the trains and prices that are perceived as being too high are the most frequent criticisms in the press. The cause is, on the one hand, that important locations are not yet connected, and on the other, the fierce competition. The Attiko Metro subway , which takes you to the city center without changing , also goes to the airport . The advantage of the connection to the Peloponnese and the north will only bring an increase in passenger numbers with the expansion. In addition, since the route network is not yet fully electrified, mainly diesel-powered vehicles are used.
In 2006 the Proastiakos saw a 30% increase in passenger numbers compared to 2005.
