In 1953, young Elise Rainier lived in New Mexico with parents Audrey and Gerald, a prison executioner, and her brother Christian . Elise claims to have seen the ghosts of the prisoners, alarming her parents. One night Elise meets a ghost in her room. Frightened, Christian looks for the pipe their mother gave them to get help, but he misplaced it. Gerald, angry, hits Elise and locks her in the basement. A demon named "Key Face" comes through, takes possession of Elise and kills Audrey by hanging her. Gerald comes and finds Elise and Audrey's body.

Decades later, Elise works as a paranormal investigator with her colleagues Specs and Tucker. Ted Garza calls Elise and says he experienced paranormal activity in his home, which is actually Elise's childhood home. She hesitates, but then accepts the investigative job.

Ted explains that a lot of paranormal incidents have happened, especially in her old bedroom. Elise finds the lost pipe in her room. In the basement she is attacked by a female ghost who yells "Help her!" Before disappearing with a pipe. Elise tells them that she saw her as a teenager, but when her father said he hadn't seen anything, Elise fled the house for fear of further beatings and gave up on Christian. The next morning, Elise, Tucker and Specs meet sisters Melissa and Imogen, Christian's daughters. Christian is still angry with Elise for leaving him. Hoping to mend their relationship, Elise gives Melissa a photo of the pipe and tells her to show Christian.

That night, Elise and Tucker hear the whistle. The sound comes from behind a wall that is actually a door. Behind the door is a bloodied girl with a chain around her neck. Ted storms into the room and it is revealed that he is holding the girl against her will. He locks the group up and tries to kill Specs, but Specs gains the upper hand and kills him by dropping a bookshelf on his head. The police save the group.

Christian and his girls go into the house to look for the pipe, where Melissa is attacked by "Key Face". The being puts two keys in Melissa: one steals her voice and the other takes her soul and sends her into a coma. Melissa is rushed to the hospital, but Imogen stays to help.

In the basement, Elise and Tucker find the red door that is the entrance to a kind of limbo . Elise also finds a nightgown that belonged to the female ghost Anna. A flashback shows that Elise had seen Anna in the laundry room. Anna was actually alive and had escaped from the room she was locked in. When Gerald came in he saw Anna but pretended not to see anything. After Elise ran away, Gerald brutally killed Anna. In the chimneys Elise finds several suitcases with the remains of women who were locked in the house. She is then ambushed by "Key Face", who takes her to limbo and leaves her unconscious body in the real world.

Imogen enters limbo, led by Anna into a prison realm where "Key Face" holds all the souls he has taken. Elise discovers that “Key Face” manipulated both Gerald and Ted by feeding on the fear and hatred of the women whose souls were trapped there. "Key Face" tries to get Elise to hurt her father in revenge for what he did. Elise starts beating Gerald, but is stopped by Imogen and refuses to feed "Key Face" any more hatred. "Key Face" attacks Elise, but Gerald saves her before he is stabbed by "Key Face" and his soul disappears.

"Key Face" stabs Melissa, causing her physical body to die. He's trying to own Elise. Elise blows the pipe and Audrey saves her and defeats "Key Face". They die with Melissa to find their corpse in limbo. You open a door and see a boy, Dalton Lambert, falling off a ladder and he sees her. When they realize they have opened the wrong door, they leave the door open and find Melissa behind the next door. Melissa's spirit returns to her body in the real world and saves her life. Elise makes up with her mother and says goodbye. Elise and Imogen return to the real world and meet up with Melissa and Christian again. Christian forgives Elise and she gives him the pipe.

In her sleep, Elise sees a vision of Dalton over whom the red-faced demon lurks. A woman, Lorraine, asks for help for her grandson Dalton - Elise helped her with her son Josh years ago - and she agrees to help Dalton.

The film received mixed reviews, with praise for Shaye's performance, but a few critics saying the franchise had run its course.

The film service judged that the film "delivers very good ghost-haunted approaches, but watered them down with a redundant dramaturgy and silly sidekicks and thus marks the previous low of the now four-part horror series".
