action

London in 1902: Sherlock Holmes leads a lonely life of boredom, consuming drugs and hanging out in opium dens . When a woman's corpse is found on the banks of the Thames , Watson tries to interest him in the case. The unidentified dead woman had a silk stocking around her neck and based on the clothing one suspects she was a prostitute. Holmes has no interest in the case and sends Watson away, but when he takes a closer look at the photo of the woman, he realizes that she is Alice Pentney, an upper-class lady. Her mother, Mrs. Pentney, is the Duchess of Narborough. The Duke of Narborough, Lord Pentney, then orders Holmes to find his daughter's murderer. Holmes finds out that the victim was anesthetized with sleeping pills in a brandy and must have known the killer.

Watson's fiancée Mrs. Vandeleur, a psychoanalyst from the USA, suspects a sexual motive and draws Sherlock's attention to the book Psychopathia sexualis . The next victim is Georgina Massingham found hanging from a street lamp. She was anesthetized with chloroform , had a silk stocking around her neck and wore Alice Pentney's clothes. Both victims had their shoes made by shoemaker Bilney. Via Mr. Bilney you come across his former employee Sarah O'Brien, who is alleged to have committed suicide. Watson has Sarah O'Brien exhumed and it is revealed that she was the first victim of the serial killer and that he had dressed the later victim Alice Pentney in her clothes.

At Georgina's funeral, 13-year-old Imogen Helhoughton is kidnapped, but the perpetrator releases her after discovering that she has a clubfoot . Holmes finds out that the Duchess of Narborough's current personal servant, Charles Allen, was employed in the homes of all three victims. However, everyone has a safe alibi for all times of the crime.

Holmes uses Roberta Massingham as bait for the perpetrator and Charles Allen is actually caught stunned her. Everyone is arrested, but Holmes explains that he must have a twin brother, which would explain his alibi. From the fingerprints they can tell that they caught the wrong one. The other twin brother kidnaps Roberta from her room. Holmes and Watson find his hiding place, free Roberta and arrest both twin brothers. Watson eventually marries his fiancée.
