Von Boeselager was accepted into the Order of Malta in 1976 and became a knight of obedience in 1985 . From 1982 to 2015 he was Chancellor of the German Association of the Order. From January 1, 1982 to March 31, 1985 he was managing director and from January 14, 1985 to April 30, 1990 honorary director of the Malteser Aid Service in the Archdiocese of Cologne . After his election in 1989 he was re-elected for five years by the General Chapter in 1994, 1999, 2004 and on June 8, 2009. In addition, Albrecht von Boeselager has been a member of the Pontifical Council for Pastoral Care in Health Services since 1990 and a member of the Pontifical Council "Cor Unum" since 1994 .

On May 31, 2014, he was elected Grand Chancellor of the Order by the General Chapter in Rome .

At the turn of the year 2016/2017 Albrecht von Boeselager was at the center of a constitutional crisis of the Order of Malta, which was triggered by his legally controversial dismissal by the incumbent Grand Master Matthew Festing .

Festing and the Cardinal Patron of the Order of Malta , Raymond Leo Burke , demanded Boeselager's resignation in December 2016, which the latter refused, whereupon Festing declared him deposed and temporarily excluded from the Order of Malta; one reason was the distribution of condoms by Maltese in Myanmar . After von Boeselager appealed to the Pope against his dismissal, which he considered unlawful and invalid , there was a partially public dispute between the Order's leadership and the Vatican about the admissibility of the five-person Vatican investigative commission, which informed the Pope about the background should. On January 25, 2017, the Vatican announced that Grand Master Festing had submitted his resignation at an audience with Pope Francis. Albrecht von Boeselager had meanwhile sued a religious order against his dismissal. In its meeting on January 28, 2017, the Sovereign Council of the Order of Malta then accepted the resignation of the Grand Master and the rehabilitation of Boeselagers, ordered by the Pope, who had to be reinstated in his previous offices and took part in the deliberations of the Council. The leadership of the German Association of the Order of Malta praised the role of the Vatican in the dispute over the Order of Malta. The Chancellor of the German branch, Stephan Freiherr Spies von Büllesheim , thanked the Pope for “helping the Order so quickly and safely in this constitutional crisis”.

The General Chapter, which met in Rome, confirmed him as Grand Chancellor for another five years in 2019.
