The three sisters Velia, Maria and Sandra Parini live in the small Italian university town of Pavia . Velia is the oldest and for the other two a role model and mother figure at the same time, while Maria is unhappily married to a television comedian and suffers from depression. Sandra, the baby girl, is an idealistic medical student who is just celebrating her 18th birthday.
The sisters suffer, each in their own way, from not knowing which way of life, professionally and privately, is right for them. Velia, who has almost come to terms with being alone, falls in love with the married university professor Massimo. But to her displeasure, her sister Maria grows into a competitor in the fight for Massimo's favor. When Sandra falls in love with a doctor, she increasingly neglects her studies. He dies in a car accident, while Sandra remains uninjured. After Massimo remorsefully returns to his wife, the rivalry between Velia and Maria turns into solidarity and together they comfort Sandra.

After success with German films such as Die Bleierne Zeit (1981) and Rosa Luxemburg (1986), the director Margarethe von Trotta moved the center of her life to Italy . Fear and Love was her first Italian film, but it did not match the success of her political films. The film was shot on the original locations in Pavia .

Fear and Love participated at the Cannes International Film Festival in 1988 in the competition for the Golden Palm , which was ultimately awarded to Bille Augusts Pelle, the Conqueror .
