Ladysmith is located in northwestern Wisconsin on both sides of the Flambeau River , which is part of the Mississippi River basin over the Chippewa River .

The urban area extends over an area of 11.89 km², which is distributed over 10.90 km² of land and 0.99 km² of water. The city is surrounded in the north by the Town of Flambeau and in the south by the Town of Grant , without belonging to one of them.

The US Highway 8 runs in west-east direction through Ladysmith and crosses the city center via a bridge to the Flambeau River. The Wisconsin State Highway 13 runs in a north-south direction through the west of Ladysmith and leaves the urban area on the southern outskirts via another bridge over the Flambeau River. All other roads are subordinate country roads, some unpaved roads and inner-city connecting roads.
