Near the bridge over the Birs , which is first mentioned around 1100, there was probably a chapel as early as the 11th century , which was dedicated to the apostle James the Elder , the patron saint of travelers.

In the 13th century, there are first records of the infirmary , which at that time was outside the city and served as accommodation for people from Basel suffering from leprosy . The infirmary and the chapel became known through the battle of St. Jakob an der Birs in 1444. After the battle, the buildings were rebuilt. In 1837, Christoph Merian acquired the church and the surrounding properties and later bequeathed them to the city of Basel as part of the Christoph Merian Foundation .

In 1894 the building underwent a fundamental change. The nave in the west was rebuilt, reusing its side wall and other individual parts, and enlarged to the east with a transept that sprawls out on both sides . A chancel was added to the east and the western vestibule was demolished. The entire interior as well as the roof and tower structure date from the late 19th century. The frescoes on the facade, created in 1917, are by Alfred Heinrich Pellegrini and show Arnold Schick's throwing stone in the battle of 1444 . A memorial plaque commemorating the battle of 1444 is also embedded in the facade .
