The machine - on the way on behalf of Trigana Air Service - was irreparably damaged. The only occupants, four crew members, were uninjured.
- On November 17, 2006, a De Havilland Canada DHC-6 flew on a charter flight from Mulia to Ilaga into a mountain around 3,200 meters high. None of the 12 people on board survived the accident. The cause of the accident is unclear.
- On February 11, 2010, the pilots of an ATR 42 from Trigana Air Service made an emergency landing in a rice field about 30 km from Sultan Aji Muhamad Sulaiman Airport after both engines had lost power one after the other. Two passengers suffered broken legs.
- On August 16, 2015, an ATR 42-300 flew into a mountain on Trigana Air Service Flight 257 from Jayapura to Oksibil in the Indonesian province of Papua on its final approach to the destination airport. There were 54 passengers on board the machine, including five children and five crew members, none of whom survived. Local residents watched the plane crash into a mountain.
