In June 2005, Brown replaced Brendan Malone as head coach of the Cleveland Cavaliers and was the second youngest coach in the NBA at the time. In 2007 he led the Cavs around superstar LeBron James for the first time in their franchise history in the NBA finals. There, however, the team lost 0-4 against the San Antonio Spurs.

In 2008, Brown was named Eastern Conference Coach of the Month for January and was selected to coach the Eastern Conference All-Star Team . This made him only the second Cavaliers coach after Lenny Wilkens who was allowed to train an All-Star team. In the 2008-09 season , Brown was honored with the NBA Coach of the Year Award .

In the following season, Brown was sacked after the Cavs were eliminated in the playoffs against the Boston Celtics . He was succeeded by Byron Scott .
