In space, the space station Athena 1 of the genetic engineering company Energyne is destroyed after the mutation of a giant laboratory rat. Dr. Kerry Atkins is the only one to survive and is able to save himself into an evacuation capsule with gene samples. The capsule explodes in space and the three sample containers plunge into the Everglades , a forest in Wyoming and the San Diego Wildlife Sanctuary. A crocodile and a wolf are infected and soon mutate into huge monsters.

The primatologist Davis Okoye, a former special forces soldier in the US Army and an anti-poaching unit who works in the San Diego Wildlife Sanctuary, has problems dealing with his fellow human beings due to poaching, but with the exceptionally intelligent albino -Gorilla George, whom he raised from an early age after the death of his parents, connects him with an unshakable friendship. You can communicate excellently through sign language . But when George inhales the fumes from the destroyed sample container, the meek monkey also mutates into a frenzied monster of enormous proportions. Soon, Okoye must discover that there are other, similarly altered alpha predators roaming northern America, destroying everything on their way. Okoye and the discredited genetic engineer Dr. Kate Caldwell, who previously worked for Energyne and hates the company, and especially the unscrupulous leader Claire Wyden who blames her for her brother's death, teamed up to find an antidote and navigate an ever-changing battlefield fight.

Claire Wyden and her brother, whose tests are used to illegally develop biological weapons, dispatch a security company they have bought, led by a man named Burke who manages to find one of the crashed containers. They clash with the mutated wolf and several of them, including Burke, are killed.

The container in San Diego is tracked down and the drugged George is caught by a government organization under the command of Harvey Russell and put on an airplane. Davis and Kate are tied up and taken away too. However, George, who is very hungry and aggressive due to his mutation, wakes up and crashes the plane. Davis and Kate survive and rescue Russell with a timely parachute jump. Now the military is also getting involved. At headquarters, in the presence of the Colonel, Davis and Kate find out that George and the Wolf are both on their way to Chicago . Claire Wyden lures them there with a radio signal on the roof of the Energyne building to kill them and sell their genetic material to arms dealers. Davis and Kate make their way to Chicago and arrive when George and the Wolf leave a trail of destruction in the city. The crocodile has also come to Chicago through the river. Davis and Kate break into the Energyne building to steal the cure that, while the size cannot be reversed, but eliminates the abnormal aggressiveness, are stopped by Wyden and her brother. Claire shoots Davis, takes Kate hostage and forces her to walk up to a helicopter platform. Davis reappears upstairs. Wyden wants George to kill Kate when he, attracted by the signal, arrives upstairs and attacks. But Kate manages to let Claire eat together with an ampoule of the medicine in George's pocket. However, the attacking monsters, including the mutated crocodile, damage the building so much that it collapses. After George reverses the polarity and returns to his normal behavior, Davis and Kate save themselves by crash landing in a helicopter. Meanwhile the military wants to bomb the city, but Russell barely prevents the airstrike. Davis and George confront the other monsters and the wolf is killed. George is seriously injured in a violent fight against the crocodile. They manage to kill the crocodile, but George succumbs to his injuries. Davis and Kate grieve, but George begins to blink and uses sign language to indicate to Davis that he was doing it for fun, much to Davis and Kate's delight. Davis thanks Harvey Russell for preventing the air strike and they now want to help clean up the city and rescue civilians.

The film is an adaptation of the video game Rampage , an arcade game developed by Bally Midway in 1986 that allows up to three players to play simultaneously. The aim of the game is to destroy houses, which is done by three monsters, a gorilla, a dinosaur and a large werewolf, who do this either by punching or kicking. Originally, the three monsters were the humans George, Lizzie and Ralph, who had been transformed.
