Ruthven is located in northern Iowa between the Little Sioux River (15 km west), a tributary of the Missouri, and the West Fork Des Moines River (16 km east), which belongs to the Mississippi basin over the Des Moines River . Lost Island Lake is about 3 km north of Ruthven .
The intersections of the states of Iowa, South Dakota and Minnesota and the states of Iowa, South Dakota and Nebraska are located on the Missouri and lie 176 km west-northwest and 178 km southwest of Ruthven.
