A first division of the country into departments was made by the legislature in 1906. At that time, a distinction was made between a part of the country east of the Río Paraguay (Oriente province) with twelve departments and the district around the capital and a western part of the country (Occidente province) under military administration.

This administrative division has been changed several times over the years. The last change to the department boundaries was made in 1992, when the Chaco and Nueva Asunción departments in the western, formerly militarily administered part of the province Occidente were integrated into the three departments of Alto Paraguay, Boquerón and Presidente Hayes.

Each of the departments listed is further subdivided into districts.
