Dr. Alex Cross is a police officer in Washington, DC After saving a woman's life as a police psychologist, he learns that his niece Naomi has been missing for four days. He then goes to Durham , North Carolina to investigate.

When Alex Cross arrives there, he sees the photos of eight other kidnapped young women in the police station . Thereupon he drives with the local police officers to a corpse that has just been discovered in the forest, which turns out to be the third victim among the eight women found dead. To Cross' relief, the body tied to the tree is not his niece. Cross takes a closer look at the appearance of the dead and realizes that the perpetrator could be a collector. This would mean that the dead were chosen by the perpetrator because of their good looks and certain character traits. A letter with the pseudonym Casanova was found on the second body .

The doctor Kate McTiernan is attacked, drugged and kidnapped by the perpetrator in her house in the evening. She wakes up in a barren room lit only by candles. The kidnapper shows Kate with a mask and explains that she is here to fall in love and to experience love. Shortly afterwards, the viewer sees "Casanova" in another room sending a photo of McTiernan to someone who knows about it. In the meantime, McTiernan contacts the other abducted women who give her their names through the board door of her dungeon.

A postcard with a dedication by "Casanova" is pushed under the hotel door of Cross. However, he does not catch the person who brought the postcard. During a meeting with his police colleagues, he discovers that the perpetrator only kidnapped women of strong character and resilience. They suspect that the women who violated certain rules of the game of the perpetrator were murdered.

Meanwhile "Casanova" comes to Kate to give her another injection with a drug. The recreational kickboxer beats him unable to act for a brief moment, finds the exit from the labyrinth-like cellar despite the drug fog and flees through the forest. However, "Casanova" catches up with her at the edge of a ravine. Kate jumps down in her fear. Cross makes his way with the other policemen to the hospital , where McTiernan, pulled out of a river by anglers, is lying. It is a post-traumatic shock diagnosed. Cross later discovers a Sistol order from Dr. William "Will" Rudolph. He then finds out with McTiernan that similar kidnappings are taking place in California and that these could be related to those in Durham. Because of this, they all fly to Los Angeles to investigate further with Cross' cousin, Det. John Sampson. There they set out to find Dr. To observe Rudolph in a bar, where he speaks to a woman and takes her to his forest hut. They follow him and try to arrest him. However, her partner Henry Castillo is shot by Rudolph while trying to escape and Cross is hit shortly afterwards. After this unsuccessful attempt, the FBI clings to Rudolph's heels and searches his house. There they find a room with clues about the second perpetrator with whom Rudolph is in competition.

Rudolph goes to the underground cellars of a dilapidated plantation, where he meets the other perpetrator and discusses excitedly with him. He calls himself his mentor , sees himself forced to shoot Rudolph and drives him away. Cross searches for the hiding place of the kidnapped women in the forest and finds it through the gunfire. Cross shoots the fleeing Rudolph, "Casanova" escapes, the women are freed.

McTiernan invites Cross to his house for dinner on his last evening in the area. He thinks about the case and realizes that the writing of "Casanova" is similar to that of Detective Nick Ruskin. The unsuspecting Kate McTiernan lets Ruskin, who is "Casanova", into her house. Cross tries to call McTiernan, but Ruskin has cut the line. Cross races to her in the car. Ruskin fights with McTiernan, who manages to handcuff him to the gas stove. Ruskin pulls on the stove and exposes the gas pipe. He wants to blow himself and McTiernan up with a lighter. Cross appears and talks to Ruskin. When Ruskin tries to light the lighter, Cross takes up his gun. He shoots through a milk carton to prevent the muzzle flash from exploding and kills Ruskin.
