The second division 1990/91 was the 74th season of the second highest Belgian division in men's football . It started in September 1990 and ended in May 1991.

The 16 clubs played against each other twice on a total of 30 match days, divided into a first half and a second half.

The 30 game days were divided into three stages: (1 to 10, 11 to 20 and 21 to 30). The table first and champion rose directly. The second promoted team was determined in a play-off round with four teams. The second in the table and the respective winners of the three stages took part. If the champion, runner-up or another club had won more than one stage, the general ranking was taken.

Since Beerschot VAV was relegated from the 1st division to the 3rd division due to financial problems, only the bottom of the table had to go to the third division this year .
