A water polo competition was held at the VII Olympic Games in Antwerp in 1920 . The venue was the Olympic water sports stadium.

The twelve teams involved played a tournament in the Bergvall system , with the USA, Greece, Great Britain and the Netherlands having a bye in the first round. After the quarter-finals and the semi-finals, the Olympic champion was determined in the final. In another knockout round, the teams that had lost to the Olympic champion played for the silver medal. The loser of this round then stepped in the bronze medal match on the teams that had lost to the losing finalist.
