At the XIX. At the 1968 Summer Olympics in Mexico City , a water polo tournament was held. The venues were the Ciudad Universitaria swimming pool and the Alberca Olímpica Francisco Márquez swimming pool.

The first six teams from the European Championship in 1966 (Soviet Union, GDR, Yugoslavia, Italy, Romania, Hungary) qualified for the Olympic tournament. Three other European teams qualified at a tournament from June 3 to 9, 1968 in Milan (Federal Republic of Germany, the Netherlands, Spain). Because of Romania's rejection, Greece moved up. The first three teams of the Pan American Games in 1967 (USA, Brazil, Mexico) were also qualified, with Mexico taking over Cuba for hosts. Also eligible to participate were Japan, winning the 1966 Asian Games, and the United Arab Republic, winning the 1965 Pan-African Games.

The first and second placed in both preliminary round groups advanced to the semi-finals, the third and fourth placed played for places 5 to 8, the fifth and sixth placed for places 9 to 12, all others for places 13 to 15.
