Sarah Marquardt is standing on the beach in front of the hotel in Krabi and looks into the distance, knowing that she must do something now. On the way to the police station, she phoned Dr. Kathrin Globisch in Leipzig to find out how Jakob is doing. Dr. Globisch would be happy if his father, Dr. Roland Heilmann could take care of him, but that is not possible at the moment because he is in prison and cannot help.

Four days before in Leipzig in front of the Saxony Clinic. Jakob is making his father a cup of coffee when he sees Eva Bühlig for the first time through the window of the cafeteria. The clinic is very busy because of a gas explosion with many injured. In the middle of the hustle and bustle, Sarah Marquardt calls Dr. Heilmann because he has not yet accepted the medical congress in Thailand. He gives her to understand that he has no time and does not want to fly to Bangkok. In the meantime, Sarah Marquardt informs Kris Haas that he will accompany her as an assistant to Thailand because she knows that he already knows the country. Jakob is shopping in town when he runs into Eva. They start talking, it turns out that Eva is currently doing her practical year at the Saxony Clinic and therefore knows that Dr. Heilmann is his father and is currently in Thailand. They arrange to meet for the next day.

While Sarah and Kris are enjoying the involuntary vacation, Roland just wants to leave. By chance he overhears a doctor's colleague talking to him where he learns that you can fly in cargo planes. He makes his way to the airport, but has to help with a medical emergency en route. At the local hospital, he runs into Helen Larsson, whom he immediately likes. But the detour took too much time and the plane is gone. The taxi driver advises him to take the bus to Malaysia the next day , from where you can fly back. He returns to the hotel to deliver the good news to Sarah.

The next morning, Sarah, Roland and Kris set off on the bus. Roland meets Helen again, who is also riding. Despite the injury, the moped driver also sets off and Helen doesn't have far to walk to her village. Roland and Kris stay behind, hoping the bus will return soon. Kris finds Roland's broken cell phone that fell out of his pocket.

In the meanwhile, the doctors at the Saxon Clinic are fighting for the life of Jakob, who has suffered cardiac arrest . He is believed to be suffering from viral myocarditis because he has carried a cold. Since water has built up, Dr. Martin Stein will likely do a heart puncture. After the procedure, Dr. Stein, Dr. Maria Weber and Dr. Establish globally that Jacob needs a new heart and should be saved with an artificial heart until then. Roland was able to organize a van from the village back to Krabi, but before they leave he wants to see Tamika again. Since Kris was still around, he helps her husband take her to the clinic. Roland is certain that she has a ruptured appendix and recommends Helen to operate immediately. Roland gives Jakob the statue he received from Helen and from then on he watches by his bed until Martin brings him the good news that the inflammation levels have dropped.

Three weeks later, Jakob is completely healthy again and goes on vacation with Eva. Roland then picks up Kris from the airport together with Sarah. “Other doctor series may be bolder, more realistic or more dramatic - but 'In all friendship' they outperform all of them. The honest doctors in the Leipzig Sachsenklinik have been on duty for 20 years, and several million fans watch the mix of patient and family stories every week - there are even several offshoots of the soap opera.

For the anniversary, ARD is showing a feature film special of the long-running hit, the focus of which is the serial hero Roland Heilmann (with Thomas Rühmann since the first episode). The honest surgeon travels to Thailand in the melodrama and is actually supposed to switch off from the stress of work there - after all, in the ideal world of 'IaF' the doctors are constantly selflessly scraping themselves for the well-being of their patients.
