Resident Meredith Gray begins her practical training as a doctor alongside four other interns at Seattle Grace Hospital. Her one-night stand from the previous night was the senior physician Dr. Shepherd out. Meredith's head of training is the strict Miranda Bailey. On her first day, Meredith treats a beauty queen. A rape victim is admitted to the hospital and has bitten off the perpetrator's penis. Her life is in danger. Meredith and Derek treat the patient. Derek is led by Dr. Bailey instructed not to favor Meredith anymore. George realizes that Meredith has a love affair and is sad. A woman with a 60 kilo tumor is admitted to the hospital. Initially, she is assigned to Alex, who is withdrawn from the case after he has spoken derogatory about the patient. Almost all doctors are ultimately involved in removing the tumor, but to no avail. The patient dies. Meredith also takes care of a Parkinson's patient who can end up walking again with the help of a chip in the brain. Since Alex negligently did not charge his beeper, one of his patients is in mortal danger. Izzie finally has to step in and perform an emergency operation in the hospital room. | |||||||

Meredith wants to learn more about his private life from Derek. At the end of the episode, he finally shows her where he lives: isolated from the big city in a trailer. An extremely devout Jew is hospitalized. She would need a pig's heart valve to survive, which is out of the question for her because of her beliefs. Alex gives her the opportunity to have a beef valve implanted. The pregnant Cristina is assigned a cancer patient who is also pregnant. She has to make the big decision of having the baby aborted or of dying soon herself. She decides to die, which Cristina has no understanding for. Izzie is assigned a clairvoyant. At first she detests him because her mother has lost her money through clairvoyants. But when she realizes that his visions are real, she believes him and gets in touch with her mother again. Preston's best friend is brought in. It is found that he has ovaries and cannot father children. Since his girlfriend is pregnant, it is certain that she was not faithful to him. Preston finally tells his friend about it. Izzie and Cristina perform an unauthorized autopsy. Although they committed a criminal offense and got into trouble from Bailey, it turns out that the patient died of an inherited disease, which Cristina and Izzie may have saved his daughter, who can still get tested and treated in time. George was infected with syphilis and therefore has a crisis with Olivia, who confesses to having slept with Alex before. That's why George hits his colleague.
