AOK Rheinland / Hamburg was created from the merger between AOK Rheinland and AOK Hamburg on July 1, 2006.
History of the AOK Rheinland
AOK Rheinland was founded on April 1st, 1994 and was based in Düsseldorf. It emerged from the following 26 general local health insurance funds:

- AOK Bonn
- AOK Düren-Jülich
- AOK Düsseldorf
- AOK health insurance for Duisburg

Finances
Contribution rates
Since January 1, 2009 the contribution rates have been standardized by the legislator. Up until December 31, 2014, the AOK did not levy any additional contribution that is independent of the health insurance fund and did not pay any premium to its insured persons. In 2015, it levied an income-related additional contribution of 0.9 percent of the contributory income. The additional contribution has been 1.1% since January 1, 2019.

The budget of the AOK is around 8.2 billion euros (as of 2014).
