The aging and dying Primo Sparazza, the last remaining godfather of a Cosa Nostra clan in the United States, is under surveillance by FBI agents Richard Messner and Donald Carruthers, who are gathering evidence to bring the godfather to justice. During a telephone surveillance, they learn that the old mafia boss has hired professional killers to eliminate a disgraced former member of the mafia - the Las Vegas magician Buddy "Aces" Israel. In addition, Primo Sparazza is not a Sicilian mafioso, but the former FBI undercover agent Freeman Heller, who was one of the first agents to investigate undercover in the mafia family in the 1940s and who later assumed that identity. Information from over 60 years of organized crime is more valuable to Locke's superiors than the life of the rather insignificant Israel, so they prompt a heart transplant. The mysterious Swede is a heart specialist who is supposed to carry out the transplant.

Messner is furious and bitter about the fact that the FBI is pretending to act for the “common good”, but does not protect its own agents sufficiently. Adequate evidence would have prevented the death of his colleague and a few other people involved. During the argument about it, Messner pretends to leave the matter behind and closes himself in tears in the sickroom where Sparazza and Israel are lying. Messner pulls out the plugs of the life support machines to which the two are connected. He then puts his pistol, his magazine and his badge on the floor of the hospital room, as a visible sign of his retirement from the FBI service. While the two patients die, several agents and doctors try unsuccessfully to break into the room.
