It has been 17 days since Cristina's broken wedding. Meredith, Izzie, Cristina and Alex are starting their second year in the hospital and are now training interns themselves. Izzie has to fight for recognition from her pupils as they have learned of her Denny past. Cristina has Lexie Gray in her group, Meredith's half-sister, with whom Meredith does not want to have anything to do with. Meredith has in her group u. a. George, who made up his mind to repeat the first year again after all. Meredith and Derek split up for good, but have sex again. Cristina learns from Derek that Preston has quit and will not be coming back. George initially avoids Izzie, but then confesses his love to her. Lexie and George become friends. Mark fights for Derek's friendship. Miranda is mad at Richard that Callie and not she became a ward doctor. Callie is overwhelmed with the new job.

Meredith still refuses any contact with her half-sister Lexie. Preston's mother has shown up at the hospital and wants to speak to Cristina. At first she avoids contact with her, but finally she lets herself into a conversation and notices that Preston's mother is well-disposed towards her and respects her. While George is considering whether to tell Callie about his love affair with Izzie, Callie already suspects this. Derek and Meredith continue to have sex after their breakup. This is actually not enough for Derek, but he cannot part with Meredith. Callie is still struggling with the new job and is not respected.

Meredith wants to spare Cristina and doesn't tell her anything about Derek. Cristina finds this consideration inappropriate. Meredith overcomes her dislike of Lexie and tells her sister the exact course of Eileen's illness and treatment. Alex gets 60-year-old Dr. Norman Shales assigned and does not know how to deal with him. He gets advice from Miranda. George is the hero of the new interns because he already knows so much and is always calm. Alex tells them that George repeats the year. George confesses to Callie his misstep with Izzie. Mark and Richard are performing a risky operation. Miranda offers Callie to support her in her job.

Derek and Meredith want to go on a short love trip this weekend. In order for Alex to take over her shift that day, Meredith has to take care of Norman, who makes a huge mistake by telling the wrong woman that she is going to die. Cristina is put in her place by Derek because she has her assistant doctors and v. a. Lexie treated disparagingly. Izzie tells Alex about her misstep with George. Alex is hurt because Izzie wanted George, but not him. Callie tries to talk to Izzie about the misstep, but Izzie misunderstands the situation and prepares to fight Callie. This creates an embarrassing situation for both sides. Callie takes her frustration out on a patient. Richard's niece is back in the hospital because the cancer has returned. But she refuses any further treatment. Derek and Lexie become friends. Derek cancels the weekend with Meredith because their relationship has no future. Although he wants to try to wait until Meredith is ready for a relationship, he cannot promise her that he will succeed.

Meredith wants to bury her mother's ashes, but she cannot find a suitable place where her mother could have felt comfortable. Finally, she and Richard scatter the ashes in the operating room sink. Cristina wants to rent Preston's apartment and first promises Richard, who is looking for a new place to live after Adele filed for divorce. Callie tells everyone about George and Izzie. Cristina cannot find the slightest understanding for the two of them. When Richard then also added Dr. Hahn hires as the new cardio-thoracic surgeon, lets Cristina move in instead of Richard Callie. Rebecca visits Alex. The two have sex, but he is not ready to talk to her about the future. Norman has had a stroke and is saved, leaving surgery. Miranda has marital problems because she puts her job above her private life. The nurses, including Olivia, allied against the Casanova Mark.

Meredith has nightmares and realizes that she needs Derek even though she can't allow him to be around. She catches Alex red-handed with her sister Lexie. Cristina suffers from the fact that Dr. Hahn prefers Izzie to her. Izzie and George finally want to sleep together, but Izzie is too exhausted. Callie decides to let go of George. Richard now lives in a trailer on Derek's property and wants to organize a men's evening with him and Mark. Finally, he also invites Dr. Hahn to do this.

The relationship between Meredith and Lexie, which had just started to improve, is strained again as Meredith does not want Lexie to meet Alex. When Alex escorts Lexie home, he notices that Thatcher has a drinking problem after Eileen's death. Izzie and George are unhappy with their sex lives and realize that the chemistry between them is not right. Callie asks Miranda to take over her job as station manager for the day. When Richard realizes that everything is suddenly going well, he removes Callie and promotes Miranda instead. Cristina, who is still under Dr. Hahn's disregard suffers, giving her apartment a new look so that it is not always reminded of Preston. Richard is struggling with bachelor life and needs help from Derek. Mark is keeping an eye on Dr. Rooster thrown, but she is not interested in him.

Meredith's father comes to the hospital heavily drunk with a cut. Meredith takes care of him and is happy when he apologizes to her and makes up with her. But Lexie brings her back down to earth by telling her that Thatcher is an alcoholic and, while drunk, doesn't know what he's saying. Derek goes on a date with Sydney Heron and meets the operating room nurse Rose. Cristina is finally Dr. Hahn was assigned, but is completely over-committed. Izzie and George hardly speak to each other anymore and want their old friendship back. Miranda has an old college mate she was in love with as a patient and realizes that he was just taking advantage of her then as now.

Two occupied ambulances collide in front of the hospital. Two paramedics are locked up and seriously injured. Meredith and Richard take care of her. Another paramedic has abdominal pain, but does not want to be treated by Miranda or Richard, but wants a white doctor. Cristina finds out that he has a huge swastika tattooed on his stomach. Miranda and Cristina, who lost their step-grandparents in Auschwitz, have to bring themselves to operate on him. Miranda drops lunch with Tucker for this, which puts their marriage to the test again. Rebecca visits Alex and Lexie finds out about them. Meredith doesn't want Derek to meet other women, but she can't tell him. Derek befriends Rose. While he is operating on the brain tumor of the driver of the accident car, the computer for the monitor fails. Lexie befriends a patient whose carotid artery bursts in her presence.

Meredith and Richard can only save one of the two paramedics in the accident car. Rose manages to repair the failed computer in Derek's operating room so that the patient can be saved. Miranda makes the right-wing radical's swastika tattoo unrecognizable during the operation. Alex is suspended for a week for letting Rebecca see an operation from the gallery. Meredith finally tells Derek that she doesn't want him to meet other women, but he has already kissed Rose in the meantime. Lexie does everything possible to save the man with the ruptured carotid artery, but the doctors fail. Cristina brings Lexie with her to Meredith and the half-siblings seem to be getting closer. George and Izzie realize their relationship will not work.

A healer is a patient in the hospital who heals other patients by the laying on of hands. Derek surprises Meredith with plans for a house together. When Meredith learns of the kiss between him and Rose, they end their relationship for good. Derek then goes out with Rose. George's mother comes to visit and learns that George has broken up with Callie. For the Christian Louise this is a sin. Mark flirts with Erica Hahn, but she wants to separate work and private life. Callie and Erica become friends for this. Miranda's son Tuck is rushed to the hospital because a bookshelf has fallen on him. He has internal injuries. Miranda and Tucker blame each other. Miranda seeks help from the healer and Tuck's life is finally saved. Tucker moves out after that. Cristina suffers from the fact that Erica still doesn't want to teach her.

Meredith, Christina, Izzie and Alex have a surgery competition led by Miranda and have been in the hospital for two weeks. Meredith wins the competition because she recognized a tumor early on and receives the so-called “twinkle pipit”, with which she can now take over the operations of the other doctors for three months. George and Lexie rented their own apartment, which is dirty and full of cockroaches. Lexie is stealing from the hospital to make the apartment more homely. Meredith wants Derek to do a research series on the healing of brain tumors. Rose and Derek are a couple. Meredith goes to hospital psychologist Dr. Katherine Wyatt, however, remains silent in the meetings.

Addison returns to Seattle Grace Hospital for a short period of time to help with difficult baby heart surgery. She is amazed at what has changed. She advises Meredith not to let Derek go so easily. Izzie is sad that George and the beginners don't want to party with her as their boss. Addison believes there is more than friendship between Callie and Erica. When she asks Callie about it, she is completely irritated and then drags Mark away. Rebecca reports to Alex that she is pregnant. At first he does not know how to deal with the situation and distances himself. Izzie gets Rebecca's blood test, which shows that Rebecca is not pregnant. Erica confesses to Addison that the reason she treats Cristina so harshly is because she reminds her of herself. Meredith and Derek's research series begins, but the first patient dies. Richard offers Addison to start again at the hospital, but Addison has taken a liking to the Los Angeles world .

The nurses' strike against Mark is getting so bad that the council has to intervene. This includes Adele, who represents the nurses. Richard hires George as his pseudo-assistant to impress Adele. Finally, Miranda stands up for Mark. All hospital staff must provide information about which hospital staff members they have slept with. The only exception is Miranda, who feels humiliated by Richard. Derek and Meredith's research series is still unsuccessful. Meredith tries to come to terms with Derek's new relationship. Derek, on the other hand, still hasn't gotten over Meredith. Alex and Rebecca make plans for the future. Cristina learns that Preston won a grand prize but didn't mention her.

Meredith dismisses her psychologist Dr. Wyatt, for alleging she was suicidal. In the end, however, she tells her that her mother once attempted suicide when Richard left her. Cristina tries to get over the disappointment with Preston. George hopes Richard will let him retake the exam. Alex realizes that Rebecca is actually just living through a bogus pregnancy and takes care of her. Derek and Rose's relationship begins to deteriorate. Miranda's relationship with Tucker continues to deteriorate. Richard and Erica operate on Richard's former professor. Erica has to make sure to perform the operation first so that if it fails, her reputation is not ruined. Erica kisses Callie, apparently to provoke Mark, who wants to give up his life as Casanova.

Meredith and Derek's study is due to be discontinued if they fail to show any success with their last patient. Her patients are a young couple who both have brain tumors. They allow them to sleep together for the first time before the operation. Mark and Callie revert to their old behavior. Doctors have to take care of a boy who has laid himself in a basin full of concrete to impress a girl. George and Lexie gain access to the training files. George learns that he only failed the exam because of a single point. Derek wants to sell his land, which Mark and Richard advise against. Rebecca, whom Alex takes care of despite her severe psychological problems, cuts her wrists. Dr. Wyatt wants Meredith to deal with her mother's suicide attempt.

Richard defends Cristina from Erica. This also makes Cristina nicer to her student Lexie. Richard gives George the chance to retake his exam. Alex doesn't want Rebecca to be treated by a psychiatrist, but Izzie can finally convince him of it. Rebecca is admitted to the mental hospital. Izzie comforts Alex and they kiss. Callie also returns Erica's kiss. Richard moves back in with Adele. Meredith realizes that her mother didn't want to kill herself. Meredith and Derek's patient dies, but they can save the girl. They want to celebrate the success of the study together. Meredith is building a house of lights for Derek on his land and wants to spend her future with him, even if she is afraid of it. The two get back together, but Derek has yet to part with Rose.
