Derek has split up with Rose, which significantly affects their cooperation in the operating room. Meredith is still unsure whether she and Derek have a future, as she constantly dreams of Derek's death. Lexie, who has fallen in love with George, learns that he has slept with her sister in the past. The hospital is ranked only # 12 of America's Best Hospitals this year. So the Seattle Grace misses out on many patients. Erica is told that she has to change her training methods. Erica and Callie avoid each other after the kiss. Military doctor Owen Hunt shows up at the hospital and attracts attention with unconventional treatment methods. At the end of the episode, Cristina slips in front of the hospital and an icicle digs into her stomach.

Meredith calls on Cristina to help her relationship with Derek. Cristina's icicle is removed by Owen and they kiss. Richard offers Owen a job as a trauma surgeon at the hospital, but Owen wants to return to the military. Alex pushes back Izzie, who still has visions of Denny. Callie has a panic attack during a procedure, but Erica skillfully calms her down. Then Erica and Callie talk about their feelings for the first time and realize that neither of them has had any experiences with the same sex. Richard announces to the assembled workforce that training methods and work ethic in the hospital need to be improved. Rose stabs Derek with a scalpel in the operating room and is then transferred to another department.

Meredith wants therapy with psychologist Dr. Wyatt quit, which she doesn't think is a good idea. Richard announces the new rules for training at a large doctors' meeting, which, among other things, no longer allow doctors to specialize in the first three years of training. A big burst pipe that floods parts of the hospital makes the new start difficult, as does George's repeat test. The test is finally supervised by Richard at the end of the day. Derek wants Izzie and Alex to move out at Meredith's. Izzie shows Cristina an advertisement because she is planning to move in with her. Cristina rents the apartment with Callie. Izzie is disappointed because she doesn't seem to have any friends anymore. Meredith makes it clear to Derek that she wants to keep Alex and Izzie as roommates. Erica isn't pleased to learn that Mark knows about her relationship with Callie.

Derek finds Meredith's mother's old diary, which throws Meredith off course. Cristina discovers the dermatological station in the hospital and is amazed at the pleasant working conditions there. George sensitively cares for a little patient who is afraid of his operation. In the evening George receives the message that he has passed the exam. Lexie wants to go party with him, but George chooses his old friends. Alex steals an operation from Izzie, but Izzie fights back. Callie and Erica are planning their first date. Callie therefore seeks advice from Miranda and confides in her. She advises her to talk to Erica about how far they want to go in their relationship. Callie and Cristina move into the new apartment.

A chain kidney transplant is due in the hospital. Doctors try to ensure that none of the patients back down. This almost happens when a woman learns of her husband's affair, but Miranda convinces her. Meredith drops one of the kidneys on the floor during the operation and makes serious allegations. She still suffers from the great success of her mother, whose diary she reads. She also dislikes the fact that a specialist journal only mentions Derek in the article about her and Derek's study. The interns argue about which doctor to hand over to George. Finally Richard makes the assignment. Lexie is angry with George for ignoring her and never campaigning to get Lexie on his team. Erica and Callie slept together, but Callie didn't like it. She then lets Mark give her sex tips. Izzie and Alex kiss.

Meredith indulges in childhood memories and finds dozens of her mother's diaries during the day. Dr. Owen Hunt, who lost his entire force on his mission in Iraq, is starting out as a trauma surgeon at Seattle Grace and giving a hands-on seminar on life-saving measures with live pigs. Izzie has a problem with that as she is against animal testing. Cristina and her assistant doctors, of whom she has a new one on her team from this day on, manage to save all the pigs with a lot of effort. Ultimately, however, the animals have to be euthanized because of their severe injuries. Alex and Izzie are back in a committed relationship. Erica raves about her night in front of Callie, but Callie still has concerns and distracts herself with sex with Mark. Callie confesses her misstep to Erica and ends her affair with Mark. Richard is very fond of Miranda and gives her the order to remove an inoperable tumor. George and Lexie make up.

Cristina is instructed by Owen to be more soulful with patients. She finally confides in him and explains her behavior. Meredith notes that Ellis' diaries v. a. are surgical reports. The patient, whose heart Izzie stole because of Denny, is admitted to Seattle Grace, and so the doctor who was treating him at the time, Erica, also learns what Izzie has done. Erica is determined to report the case. She gets into an argument with Callie about this. The interns compete for the first solo operation. On that day, Izzie keeps seeing Denny's appearance. Miranda and Tucker try to save their marriage with marriage counseling. Lexie finds out that the beginners practice stitching on themselves.

Erica quit the hospital and left town without saying goodbye to Callie. She finds it difficult to process the separation. Mark supports her and stands by her. Cristina learns from Callie that she and Erica were dating. Erica's job is now to be carried out by the new doctor Dr. Dixon, who doesn't feel comfortable in the hospital. Meredith's college friend Sadie starts working at the hospital as an intern. Cristina is jealous of Sadie. Sadie also joins the self-operation attempts of the interns around Lexie and her friends in the hospital, but the attempts are discovered and banned by Cristina. Izzie is accompanied by Denny's illusion all day. She talks to him and finally they kiss. Meredith fears that Mark is into Lexie and asks Derek to forbid him. Owen kisses Cristina again.

Callie is knocked down by a patient who is having a panic attack and is hospitalized with a broken nose and a concussion. In spite of Cristina's ban, the beginners continue with their self-experiments. When they try to remove Sadie's appendix, the operation becomes complicated and the beginners have to call their instructors. Sadie can be saved, but all beginners and Cristina get a temporary surgery ban. Cristina and Meredith get into an argument because Meredith did not defend Cristina from Richard. Izzie spends the night with Denny and the lines between reality and fiction are blurring for her.

Cristina is unanimously chosen by the senior physicians for the solo operation. Because of her surgery suspension, she has to choose one of her colleagues to take over the surgery instead. Her choice falls on Alex, which Meredith takes very personally. Izzie neglects her clinic and spends all of her time with Denny. George notices that something is wrong with her. Alex chooses Izzie to be his surgical assistant and confesses his love to her. Dr. Dixon is coming to Seattle Grace again but wants a better deal. Sadie flirts with Callie, who is unsure how to feel about it. Both Mark and Lexie and Cristina and Owen get very close.

It's Izzie's birthday and tells Alex that she can see Denny. Alex reacts surprisingly calmly and interprets Izzie's illusions as part of coming to terms with past events. The boy Jackson is Presscot for renewed bowel resection hospitalized. When his pediatrician dies of a heart attack, Richard finds Robbins' replacement in Arizona, with whom Miranda does not get along well at first. Richard worries more and more because he believes he did not run the hospital properly. Derek, Meredith, Owen and Cristina have serial killer William Dunn as patients. Derek doesn't believe in treating this patient like any other patient, and in doing so comes into conflict with Meredith. He then tells her that his father was murdered. Mark regrets his one-night stand with Lexie, but in the evening he - unlike Callie Sadie - cannot resist her again. Cristina and Meredith still haven't reconciled.

Derek's mother comes to Seattle Grace to visit Derek. Meredith first plays her the perfect daughter-in-law, but finally she confesses her mistakes. Derek's mother likes Meredith's honest manner and she gives Derek an engagement ring because she thinks Meredith is the right person. William Dunn doesn't want to die by the death penalty, but in hospital. He also wants to donate his organs to Jackson. Derek does not want to allow him natural death. However, Jackson absolutely needs the organs, as complications have arisen when inserting the long-awaited donor organs. Meredith tells William that a severe shock in his head would cause a cerebral haemorrhage, resulting in death. William then makes a suicide attempt. Owen asks Cristina out on a date, but then comes to her home late and completely drunk. Cristina learns that he is still being haunted by the events in Iraq. Izzie breaks up with Denny.

Williams' health is deteriorating rapidly. So that he can donate his organs to little Jackson, Meredith tries to hide this from Derek. William finally panics and still wants life-saving surgery. A few days later, Meredith goes to William's execution and is completely upset afterwards. Cristina overcomes the argument between the two of them and comforts Meredith. Miranda and Richard jeopardize their medical license as they battle to get Jackson new organs. Another donor gave him the transplant at the last minute and could be saved. Mark ruptured his penis while having sex with Lexie , which Callie and Owen treat. To divert the other interns' suspicions from Lexie, Sadie claims it was she who slept with Mark. Izzie is amazed that Denny is still there despite their breakup. She eventually learns that he is with her because Izzie is seriously ill. Izzie finally breaks away from him and wants to fight for her life. Cristina, who told Derek about his proposed marriage proposal, gives Owen a second chance.

After Derek is initially unsure whether Meredith is ready to propose, he convinces himself during the day and prepares her room decoratively. Before Meredith arrives, however, he receives a call from Addison, whereupon he postpones the marriage proposal. Owen has a panic attack when he sees a woman in the hospital. Cristina helps him calm down. Izzie has various tests done to find out about her illness. But your blood samples only show a harmless anemia . Callie is depressed from her single life. She gets to know Arizona, from which she is kissed. Richard tells Miranda that he will soon promote her to senior physician. After the Jackson incident, Miranda no longer wants terminally ill children as patients, but Arizona advises her to switch to pediatrics . Dr. Dixon operates again at Seattle Grace. Lexie stipulates that Mark only wants to be with him if he reveals their relationship to Derek.

Addison comes to Seattle with her brother Archer because he has to undergo emergency surgery because of worms in Derek's brain. Also there are Naomi and Sam Benett, good friends from Los Angeles. Archer's life can be saved by Derek and together with Mark they reminisce about their time together. The interns take part in a competition organized by Izzie, which Lexie wins. However, through the competition George also finds that Sadie lags far behind the other beginners in her level of knowledge. After talking to Richard, Sadie quits. The woman Owen escaped from turns out to be his former fiancée. He confesses to Cristina that his acquaintances do not yet know of his return from Iraq. He doesn't want to show them that he's not the same anymore. With Izzie, the first disorders of perception and coordination appear. Mark and Lexie flirt, but she doesn't take back her condition.

Addison extends her stay in Seattle for an operation. During the operation, Derek makes a mistake and complications arise. He gets into a momentous conflict with Addison and is desperate when the patient dies. When Mark also confesses to him that he is meeting Lexie, the whole thing ends in a fist fight. Miranda is disappointed with Richard for handing her a run-of-the- mill recommendation letter for her application as a pediatrician. Addison wrote her a very positive letter for this. After Arizona learns that Erica was Callie's first girlfriend, she is no longer interested in a romance. Callie seeks her out in Joe's bar to persuade her, but she bursts into a date from Arizona. Izzie learns that Sadie had switched her blood sample. Instead of anemia, she shows evidence of cancer that she is trying to investigate. Owen shows a lasting interest in Cristina.

Derek is being sued for possibly making a mistake with the deceased patient. He falls into a great depression and isolates himself from his private life and his job. Richard tells Meredith that Derek wants to get engaged to her. When Meredith comes to Derek with this information, he throws the ring away from her eyes. After a clarifying conversation, Izzie and George become friends again. Izzie leaves from their junior doctors anonymously make a diagnosis and learns that she is a melanoma with metastases has. They only have a few months left and their chance of survival is only 5 percent. Izzie confides in Cristina. Alex earns praise from Arizona. Callie and Arizona go out on a date. Owen wakes up and injures Cristina. He then wants to distance himself from her, which she does not allow. Richard and Miranda get into an argument about their desire to switch to pediatrics . Miranda calls Adele to settle the dispute.

Cristina has her first solo operation. Since Izzie does not want to be treated, Cristina does not know what else to do than to tell the others about the cancer. Callie and Owen try to get Derek to return to the hospital. Richard does not succeed either. Derek is unsure if Meredith could forgive him. Meredith tells Derek to go back to his job to save Izzie. Lexie admits her relationship with Mark at the hospital. Meredith and George want to find out why interns Pierce, Steve and Megan are acting so strangely. It turns out that Megan slept with Steve on a break from Pierce's relationship and is pregnant.

Derek returns to the hospital and Izzie's brain surgery is due. None of her friends care about her. Callie blames herself for wishing Izzie to die during the time of Izzie and George's affair. Derek is very excited about the operation. He proposed to Meredith twice before, but she refused to answer each time. The operation succeeds and it turns out that Meredith believed Derek from the start because she had suggested that Izzie retrieve eggs before the first radiation treatment . Owen has a nightmare and almost strangles Cristina in her sleep. Only Callie can prevent worse. Since Cristina no longer feels safe in Owen's presence, she ends her relationship with him. Derek, however, finally makes the long-awaited marriage proposal to Meredith in the elevator, which she accepts.

Izzie is busy planning Meredith's wedding, but things get worse and worse as the day goes on. Derek and Mark are still fighting over Mark's relationship with Lexie, but they eventually reconcile. Lexie is happy that Meredith wants her as a bridesmaid. Owen goes into psychotherapeutic treatment with Dr. Wyatt. At first he cannot talk about his feelings there. Miranda does psychological work in pediatric surgery for a dying child and his father. Callie's father visits and, after learning of Callie's relationship with Arizona, wants Callie to leave Seattle Grace. Callie refuses and therefore has to forego financial support from her father. Owen suggests that George specialize in traumatology .

Izzie's mother visits Izzie in the hospital. With the help of Miranda, Izzie lies to her mother that her condition has improved so that her mother will leave her. But the truth is, Izzie's condition has become more critical. Thatcher is released from rehab and wants to apologize to his daughters. Meredith doesn't want to forgive him. Richard interferes and afterwards regrets to her that he did not stand up for her in Meredith's childhood. Thatcher meets Mark at dinner with Lexie. Cristina feels ignored by Owen. He explains to her that he wants to protect her. His psychologist wrote down three-word sentences for him that he can say to her instead of a declaration of love. Callie has run out of money because of the argument with her father. Her friends recommend her to lie, but she wants her family to accept her for who she is.

On Derek and Meredith's wedding day, George, Callie, Owen, and Lexie are in the emergency room with a host of young people who were injured on their way to their graduation ceremony. George takes it very much to see the youngsters die. Derek gives Meredith a solo operation for her wedding. Izzie is hallucinating again, and with the help of Denny, Derek and Miranda detect an inoperable tumor. With this information Derek and Meredith decide to give their wedding, which was planned by Izzie anyway, to Alex and Izzie, who are spontaneously married. In the evening Izzie notices that her hair is falling out. She decides to go bald now. When Denny's hallucination comes to her again, she finally rejects him. Cristina refuses all Owen advances. Callie and Arizona get together after a failed date.

Izzie is faced with the big decision of whether to refuse an operation or have the tumor removed. During the operation, she could not only lose memory and language, but also her personality. She finally decides to have the operation, but does not want any life support afterwards. With the help of Cristina, Owen finally manages to visit his mother. After a patient who raves about the Army, George signs with the Army as a medic. Both Richard and Arizona vie for Miranda to choose her specialty. Meredith and Derek want to get married as soon as possible. Mark asks Lexie if she would like to move in with him, but is put off by her until later.

Izzie survived the operation, but she lost her short-term memory. Alex practices with her all day until she can completely remember. Shortly thereafter, her heart stops. Against Izzie's advance directive , doctors try to revive. Cristina tells Owen that she loves him and wants to risk the relationship with him. Tucker gave Miranda an ultimatum. She opts for general medicine and for divorce. Derek and Meredith seal their marriage with a contract on a Post-It . George wants to start in the army the next day. All of his colleagues except Arizona met with incomprehension. When George leaves the hospital, he saves a woman from being run over by a bus and instead comes under the wheels himself. George's face is completely disfigured so that no one recognizes him. He can identify himself to Meredith, but his heart also stops working during his operation. In the final scene Izzie can be seen getting into the elevator in the dress she was wearing on the day Denny's death. When the door opens again, George is waiting for her there in his army uniform. It is not clear whether either will survive.
