Batman vs. Two-Face is a cartoon about the cartoon character Batman and is, after Batman: Return of the Caped Crusaders, the second animated film based on the real-life series from the 1960s named after the title character . This was the last film Adam West starred in before his death.

The scientist Hugo Strange invented the malignancy extractor, a machine that is supposed to suck out evil in gas form from humans. Attending the premiere are Batman , Robin and District Attorney Harvey Dent, a good friend of Batman's alter ego Bruce Wayne. When Strange tried the machine on the super villains Joker , Penguin , Riddler , Egghead and Mr. Freeze , they overloaded her with their malevolence. There is an explosion in which Harvey is hit in the face by the collected gas and becomes the malicious Two-Face . After a long wave of crime, Harvey can be cured by plastic surgery. He invites Bruce, of whose double life he has no idea, to a fundraiser.

When Batman and Robin once again arrest King Tut, a historian with a split personality who believes he is a pharaoh, his prey and his escape vehicle, a double-decker, have disappeared. Shortly thereafter, the police received a lead on a new crime committed by the villain Bookworm, who wants to steal valuable books from the library. When he was arrested, he claims that he did not send a notice to the police. This time, too, the prey disappears. Batman and Robin suspect that due to the fact that Tut took a double-decker and that every book Bookworm tried to steal has to do with the number two, Two-Face could be behind it. But Batman doesn't believe in it, after all, Harvey is healed. In his old hiding place, Batman and Robin actually meet a man whose right half is hidden in the shadows, but actually appears to be Two-Face. He almost manages to defeat her in a fight, just the decision by flipping a coin makes him leave her alive. Batman and Robin visit Harvey, who claims not to be aware of what has happened. Robin remains suspicious. Shortly after the two superheroes left, Harvey speaks on the phone to a stranger whose voice sounds like Two-Face.

Batman and Robin quarrel, as Robin believes Dent is guilty. Batman sends Robin to his room, but he secretly pursues Dent into a warehouse, where he is sprayed with the gas from Strange's machine by Two-Face and Hugo Strange, who had been released after the failed experiment. Shortly afterwards, Batman tracks down Robin in the city, where he hits cars and windows with a crowbar. He has the same evil half of the body and split personality as Two-Face. Batman can overpower him and give him an antidote.

Their research leads the dynamic duo to a casino, where they finally find themselves tied to a giant coin on a catapult. Dent now reveals the truth to them: although his face was restored by surgery, Two-Face was the result of the malignancy extractor, whereby the evil still lived in him. Dent had turned into Two-Face over and over again and committed his crimes. The fundraiser had been a trap for Bruce Wayne to find out if it was Batman, as Bruce would never miss anything like that unless he was prevented from being Batman. Since Bruce did not show up for auction, Two-Face's suspicions are confirmed. For absolute certainty, he unmasked Batman, but puts the mask back on to sell the secret of Batman's identity to the highest-bidding supervillain. Catwoman, who has since escaped from prison, interferes to save Batman. However, the other villains outbid them by simply pooling their money. Batman triggers the catapult, which would actually cause the coin to land on a pile of iron spikes, but he and Robin can shift their weight to one side so that the coin falls on its edge and rolls on. After a fight against the villains, they discover that the real purpose of the auction was to extract more evil essence from the bidders with the help of Strange's machine, which Two-Face is now trying to spread with the help of the stolen flyer in order to turn all citizens into Two-Faces.

Batman and Robin chase Two-Face in their Batmobile and eventually crash his plane in a factory. Batman faces Two-Face alone. He strangles Dent and now completely transforms into Two-Face, so that the evil half can now be seen on both sides. He's about to kill Batman, but Batman persuades him to toss the coin. But as often as he tosses, the coin always remains empty. Now Harvey is regaining control. He beats himself up and thus Two-Face and finally ousts him for good. He then collapses unconscious. By plane, Batman sprays the antidote over Gotham. Batman learns from Commissioner Gordon that Catwoman has disappeared and explains why Two-Face couldn't make up his mind: Batman had secretly exchanged his coin for one that is empty on both sides.

Months later, Harvey Dent forgot Batman's secret and is holding a stag auction at Wayne Manor. The first bachelor he auctions is Batman. The first commandment comes from Catwoman.

A deleted scene on the Blu-Ray version shows how Dr. Quinzel, who assisted Hugo Strange in his experiment to rescue the Joker from prison under her new name Harley Quinn .
