The women's 400-meter hurdles at the 1992 Olympic Games in Barcelona was held on August 2nd, 3rd and 5th, 1992 in three rounds in the Barcelona Olympic Stadium. 27 athletes took part.

Olympic champion was Sally Gunnell from Great Britain. She won ahead of the two Americans Sandra Farmer-Patrick and Janeene Vickers .

Heike Meißner and Silvia Rieger started for Germany . Rieger failed in the preliminary round, Meißner was eliminated in the semi-finals.
Athletes from Switzerland, Austria and Liechtenstein did not take part.

Three athletes from the united team and two US women qualified for the final. The final field was completed by one starter each from Jamaica, South Africa and Great Britain.

A duel between the world and European champion Tazzjana Ljadouskaja and the world's best of the year Sandra Farmer-Patrick was expected. The British Sally Gunnell was also rated as runner-up world champion.

In the final, Farmer-Patrick took the lead on the third hurdle. But after the ninth hurdle, she lost her rhythm. The South African Myrtle Bothma fell in the finish curve and had to give up the race. At the start of the home stretch, Farmer-Patrick and Gunnell were tied at the top. The American Janeene Vickers followed just behind, but now she slacked off and had to let her two competitors go. Fourth was initially Wera Ordina, who was at the start for the united team. Behind her, Lyadouskaya got her second breath and made up a lot of ground. Only Gunnell and Farmer-Patrick fought for gold. Sally Gunnell was strong in the end and won 46 hundredths of a second ahead of Sandra Farmer-Patrick. The finish photo had to decide whether to win the bronze medal. Janeene Vickers knew Tazzjana Ljadouskaja's attack could not be fended off with the naked eye. Vera Ordina was fifth ahead of Margarita Ponomarjowa, who also ran for the combined team.
