Three US women and two Cubans qualified for the final. The final field was completed by one starter each from Bulgaria, Greece and the United Team.

Since the reigning world champion Ludmila Naroschilenko could not start in the semifinals due to injury, the US-American Gail Devers, who had already become Olympic champion over 100 meters here in Barcelona , was the sole favorite. The Bulgarian Olympic champion from 1988 Jordanka Donkowa and LaVonna Martin from the USA were also rated highly .

In the final, Devers was in the front up to the ninth hurdle. When she touched the tenth and final hurdle, she stumbled and completely lost her rhythm. She managed to finish fifth, but of course she could bury her hopes of a second Olympic victory. Four hurdles passed her and the gold medal was surprisingly won by the Greek Paraskevi Patoulidou, who hadn't been on the bill before. LaVonna Martin came second and Patoulidou's predecessor Jordanka Donkowa won the bronze medal ahead of the third US American here in the final Lynda Tolbert. Sixth was the Cuban Aliuska López.

Paraskevi Patoulidou was the first Greek woman to win a medal in the Olympics. At the same time, it was the country's first Olympic athletics victory since 1912 .
