Warren Cawley, often referred to as "Rex" - "King" - was the world record holder and at the same time the clear favorite. However, he was struggling with a recurring injury from the previous year, tissue inflammation after a muscle tear. A second American - James Luck - also went into this final with a severe handicap: He was affected by a swelling of the lymph glands that significantly weakened his performance. In the interim, the third American, Billy Hardin, son of the 1936 Olympic champion Glenn Hardin , was eliminated.

Cawley started the final race in a very controlled manner. At the seventh hurdle he put the leading Italian Roberto Frinolli and took over the lead. With a lead of half a second, Cawley ran to the safe Olympic victory. There was an exciting duel for the silver medal between the Briton John Cooper and Frinolli's compatriot Salvatore Morale, which the Briton won when they were tied. The Olympic record of 1960 continued after these games.

Salvatore Morale won the first Italian medal in this discipline.

In the 13th Olympic final there was the eleventh US victory. It was the sixth victory in a row by a US runner.
